Available shifts - Monday - Friday between 8:00 am - 5:00 pm - Nights (until 8:00 pm) and Saturdays by appointment only

As a Clinician, you will work as part of an interdisciplinary team with the underserved populations in our community by providing clinical counseling to individuals, families and couples in an outpatient mental health clinic licensed by the New York State Office of Mental Health (OMH). You will report to the Director of the Behavioral Health Center. Sessions may be conducted in the office or via Tele-mental Health as clinically indicated. This is an on site position

Responsibilities:

• Manage a caseload of clients aged birth to 65, providing comprehensive assessment, diagnosis, and individual and family therapy services to those assigned to caseload.
• Create initial treatment plans, subsequent treatment plan reviews, progress notes for each scheduled appointment, and other documentation/assessments as indicated.
• Collaborate with on-site prescriber, other medical and mental health providers, schools and any other community supports for client care.
• Attend Behavioral Health Center staff meeting twice a month.
• Attend and participate in Supervision; weekly, bi-weekly or monthly based on licensure level or individual needs.
• Provide referrals and linkages to community supports, resources, and services.

Requirements and Education:

• Master's degree in Social Work or Mental Health Counselor.
• Experience in human services, case management, line staff at a residential program, or any other related direct care within human services, both pre and post master degree, will be considered.
• License and/or Limited Permit eligible, have a limited permit in discipline or Fully Licensed (LMHC/LCSW/LCSW-R), preferred.
• Good standing with the NYS Office of the Professions.

Description

As a Clinician for Mobile Crisis, you will provide crisis risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), crisis stabilization and interventions, de-escalation techniques and follow-up services to the identified client experiencing emotional, behavioral, and psychiatric crises. You will provide these interventions by telephone and on-site mobile visits and will implement them using a strength-based, trauma-informed, systems-oriented perspective, including the development of crisis prevention and safety plans; follow-up to evaluate the effectiveness of plans; coordination of services with existing treatment providers; and linkage to additional community-based therapeutic resources and supports. You will directly report to the Clinical Site Supervisors with Director and Executive Program Director guidance. You will work on-site with travel to the community and individuals' homes.

Responsibilities:

• Provide rapid response to adults, children, adolescents, and families, who are in crisis, experiencing an emotional or behavioral disturbance by phone and on-site within the community, and as assigned, participate in acute care emergency services rotation system designed to facilitate and strengthen community-based crisis intervention services. This includes rotation of schedule to provide coverage during holidays and weekends.
• Conduct risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), mental status examinations, and substance abuse screening.
• Analyze information to identify action plan, which includes crisis stabilization plan, safety, and crisis prevention plans, and plan for follow-up. Consult with team and supervisors.
• Coordinate with community resources for collateral input and follow-up care, including referrals for mental health treatment and substance abuse assessment and treatment.
• Provide short-term crisis intervention and stabilization services to adults, children, adolescents, and their families in the home or community (wherever the individual is experiencing a crisis).

Schedule/Shift:

• Fulltime; 5 days a week 8 hour shifts or 4 days a week 10 hour shifts
• Weekend and holiday expectations
• When scheduled for a weekend shift your schedule will be 5, 8 hour shifts for that week

Requirements and Education:

• Masters' degree (MSW/MHC) and two years of related experience required
• LMSW, LMHC, LCSW, LCSW-R, MHC – LP or MSW-LP licensure required
• MSW -LP holders must be license eligible and willing to apply for a limited permit upon hire.
• MHC- LP holders must already have obtained their limited permit and be close to finishing required clinical hours
• Willing to pass and obtain licensure within 3 months of hire - Required
• Must have a valid NYS Drivers License and be willing to travel to clients' homes and communities under a variety of circumstances and weather conditions.
• Must be willing to work in many settings, including clients' homes and other community locations where there may be pets, cigarette smoke, and other irritants.
• Rotating weekend and holiday shifts required

As a Residential Youth Counselor, you will provide daily therapeutic care and programming according to agency policy and governing bodies' (DSS, OMH, JCAHO, etc.) requirements for elementary aged children.. You will work on-site on our Albany campus and will report to the Residential Supervisor.

Responsibilities:

Provide for the physical and psychological needs of clients, including physical safety, hygiene, clothing, food, cooking, medical care, and providing a reliable and nurturing environment.

1:1 Residential Counselor

• You will follow an individual youth’s 1:1 expectations plan, ICMP, and any safety plans in place.

• Engage the youth in activities and conversation that are age appropriate and at the appropriate developmental level.

• Document in writing a summary of each shift.

• Learn the history and specific issues associated with the youth who requires 1:1; will learn how to safely manage any crisis.

• Seek and obtain training in any particular area specific to the youth who requires 1:1care.

• Transport 1:1 client for medical, mental health, and other essential needs.

RC1

• You will must be able to work within trauma informed framework of program, including identifying and using coping/safety plans, maintaining consistency, providing regular routine, and controlling own response to youth.

• Engage and form positive relationship with all youth served.

• Complete all mandatory trainings according to programmatic guidelines.

• Display basic competency in TCI techniques.

• Follow outlined program routines, therapeutic plans of youth, and program rules and protocols.

• Work as part of a team including colleague residential counselors, clinicians, on line oncall, supervisors, and other administrative and functional support staff.

• Maintain respectful and appropriate communication with internal and external customers,including county workers, medical staff, parents etc.

• Complete and maintain documentation to meet programmatic and regulatory standards.

• Transport clients for medical, mental health, and other essential needs.

RC2

All of the above plus the following:

• You will facilitate groups/coached visitation, understanding and applying curriculum and appropriate practice models, i.e. DBT, SAY, etc.

• Take on specific leadership responsibilities for dorm/cottage, including coordinating activities, participating in program-wide committee.

• Must have the ability, skill, and knowledge to take a lead role in a team.

• Form positive relationships with more challenging youth and families.

RC3

All of the above plus the following:

• Display critical thinking and problem solving skills related to residential program needs.

• Display good judgment and decision making related to all program routines, therapeutic plans of youth, and program rules and protocols.

• Lead, delegate, and plan work for other team members.

Requirements and Education:

• HS Diploma/GED required.

• Experience working with emotionally disturbed population.

• A valid, clean driver's license is required.

• Must pass physical.

• Must be able to complete TCI training.

RC 1

Experience with emotionally disturbed population is preferred.

RC 2

At least six months of related experience is required with demonstrated competency in Level 1 essential job duties. One year of related experience is desired.

RC 3

At least one year of related experience is required with demonstrated competency in Level 2 essential job responsibilities. Two years or more related experience is desired.

As an employment specialist for the ACT program, you will support the employment needs for ACT enrolled participants in Warren, Washington and Saratoga counties. You will help participants to identify interests and skills related to employment. You will provide services including preparation for employment, job training, developing community relationships with potential employers or vocational agencies, and educating community employers about serious mental illness. You will report to the Director of ACT and will work onsite with community travel.

Responsibilities:

• Engage individuals and establish trusting, collaborative relationships directed toward the goal of employment in community job settings

• Provide job development and job searching services directed toward positions that are individualized to the interests and uniqueness of the people on their caseload, following the procedures of the individual placement and support approach to supported employment

• Prioritize vocational skill and independent living skill development during routine client sessions.

• Provide individualized support to assist participants in securing and maintaining employment

• Support client needs surrounding insurance, identification, additional income, and other related needs to encourage independent living and personal success.

• Arrange access to benefits counseling and additional supports needed.

• Share a caseload of approximately 48 adults (residing in Saratoga, Warren and Washington Counties) with the ACT team members; responsible for the oversight and completion of their treatment planning, clinical documentation, charting, and all necessary OMH, ACT, and Agency protocols.

• Participate in daily meetings surrounding case reviews, crisis intervention, clinical formulation, treatment planning and additional client/program needs.

• Participate in the agency trainings, that in turn support our client's needs, as assigned by Director • Must be willing to transport clients, in the agency vehicles, when applicable/appropriate.

• Meet with employers to provide psychoeducation about serious mental illness and develop plan on how to best support client's and employer's needs.

• Other tasks as assigned.

Requirements and Education:

Bachelor’s degree, OR high-school diploma/ GED, and have at least six (6) months of direct care experience with individuals with serious mental illness

A valid NYS driver's license required.

As the Behavior Support Classroom Specialist, you will ensure the safety of the students in the Alternative Programming Room. You will coordinate clinical, academic, and restorative efforts for students in APR. You will report to the Dean of Students and will work onsite.

Responsibilities:

• Coordinate daily schedule for Alternative Programming Room with teaching staff, classroom staff, and clinicians.
• Coordinate Restorative Practices efforts with the administrative team
• Communicate with leadership team regarding incidents and interventions.
• Support and correct behavior support staff interactions and interventions with students.
• Ensure that APR support staff follow school and agency procedures while working with students.
• Coordinate with treatment teams and families regarding behavioral concerns that are affecting the milieu and academic programming.
• Help design and implement treatment services for children in collaboration with students, educators, administrators, and clinicians.
• Assist and educate staff with implementation of crisis intervention work, conflict resolution, and mediation strategies throughout the school.
• Be a school liaison with residential staff to ensure consistency of care and support of students across programs.
• Participate in implementation of PBIS Tier II and Tier III student plans.
• Collaborate with all members of the school and clinical treatment team to ensure ongoing communication and consistency of interventions and treatment for students.
• Ensure that all reporting forms are completed and returned to the appropriate source.
• Maintain a high level of confidentiality regarding student information observing agency, state, and federal regulators.
• Maintain CPR and First Aid certifications.
