Responsibilities:

• Arrange and attend home visits

Requirements and Education:

• Requires a HS Diploma/ GED
• Experience with individuals in care systems preferred.

As a Teacher aide, Substitute, you will assist the classroom teacher in all aspects of behavior management and supporting students success in school. You will report to the Principle and you will work onsite.

Responsibilities:

• Follow all Agency policies and procedures, along with state and federal regulations pertaining to our school and students.
• Monitor student behavior and help maintain discipline in the classroom; record behavior data as requested; assist with crisis prevention; assist with de-escalation techniques with students.
• Assist in the implementation of behavior plans (BIP); record behavior data; and the completion of incident reports when requested.
• Monitor, supervise and assist students during non-instructional periods within a variety of school environments (e.g. lunch, recess/breaks, bus loading/unloading, playground, classroom, washroom, field trips, transportation, assemblies, etc.) for the purpose of maintaining a safe and positive learning environment.
• Assist in the implementation of classroom PBIS universal components and Tier 2 and Tier 3 strategies for selected students.
• Attend meetings, training, professional learning sessions (e.g. first aid, TCI, CPR, emergency procedures, behavior management, etc.) for the purpose of enhancing professional skills and competencies.
• Perform various clerical duties; maintain records of student progress; develop and file incident reports; maintain student files; check daily attendance; make copies; develop classroom displays and instructional material; maintain and operate computers, smartboards and other classroom equipment.
• Collaborate with teacher and other staff to incorporate information provided by parents, doctors, nurses, therapists and others in order to understand the needs of and provide services for students.
• Always monitor safety and well-being of students; monitor student attitudes and encourage positive behavior and interactions with peers.
• Perform other related duties and responsibilities as requested by the teacher and principal

As a Case Manager, you will work with the children in care, the Department of Social Services, foster families, and birth families in the journey toward achieving permanency in multiple settings. You will report to the Program Coordinator and work on-site with some travel.

Responsibilities:

• Work with foster and birth families to coordinate all appointments for children, including providing transportation if another plan cannot be found.
• Attend all client-related meetings, including Counsel on Special Education, Termination of Parental Rights, staff meetings, service provider meetings, emergency meetings, court, and others.
• Facilitate meetings with other service providers.
• Support birth families including providing supervised visitation and feedback on parenting skills and communication regarding progress/problems of children in care and obstacles toward permanency.
• Participation in treatment and discharge planning.
• Regular case documentation.

Requirements and Education:

• Associate's Degree and two year of experience in social services, or equivalent education and experience.
• Ability to work night and weekend hours and be part of an on-call rotation to support foster families in after-hours crisis.
• Driver's license and reliable, safe, and clean transportation and a willingness to transport clients in own vehicle.

POSITION SUMMARY

Provide targeted skill building activities with youth in community residence program. Skill training to include areas of independent living and social skill activities.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• At minimum, 50% of work week is spent face to face with clients to provide services designated in the Treatment Plan.
• Provide services based on the youths needs and preferences as identified in the Treatment Plan.
• Work with treatment team members to identify goals and objectives of treatment, including youth, families, school, and counselors.
• Complete all required documentation and submit to the Supervisor within required timeframes.
• Treatment plan goals may include Independent Living or social skills training examples Career Planning, Self-Care, Daily Living, Housing, Money Management, including establishing and maintaining interpersonal relationships, how to accept no as an answer, handling conflict with peers and authority figures and using their coping skills when feeling dysregulated.
• Staff need to maintain consistency, provide regular routines and be aware of their own emotions when dealing with the youth in care.
• Engage and form positive relationships with all youth served.
• Maintain respectful and appropriate communication with internal and external customers, including county workers, medical staff, parents etc.
• Develop and maintain relationships with community organizations and consumers to expand opportunities for community connections.
• Facilitate service provision and attend relevant meetings as required.
• Provide information to collateral contacts for the purpose of Treatment planning and assist in tracking progress as required.
• Attend all required trainings and meet regularly for supervision.
• Ability to work with diverse consumers who have mental health challenges and may be developmentally delayed. Understand their independent living needs to work with case manager to find resources in the community for educational and vocational skills and programming for discharge.

EDUCATION and/or EXPERIENCE

• High School Diploma/GED required
• At least 2 years of experience

Clinician, Residential

Full-time Licensed Social Workers are eligible for a $7,500 retention bonus!

Would also consider per diem of 3-8 hours per week

As a Clinician, you will coordinate client assessments and work with the child, family, legal guardian, law guardian, psychiatrist, nurse, psychologist, educators, child care staff, and other providers to prepare, implement and monitor a comprehensive, permanency-focused case plan for children in the Residential Program. You will report to the Director of the Residential Treatment Facility and will work on-site.

Responsibilities:

• Work with the direct treatment providers to establish treatment goals for children, utilizing historical and current available information.
• Meet and communicate with biological families, guardians, foster families, and case workers.
• Assess individual and family situations.
• Identify problem areas, treatment goals and methods to attain goals.
• Provide therapeutic services to children on at least a weekly basis and families on at least a bi-weekly basis using differential treatment modalities.
• Work with interdisciplinary team to develop and implement a comprehensive treatment plan and to monitor ongoing progress of child and family.
• Learn and practice case coordination skills.
• Work with referring and collateral agencies and prepare reports and correspondence as needed.
• Record and maintain case record in accord with Agency, OMH, and other state mandates.
• Participate in intake assessments as assigned.
• Diligently plan for discharge/permanency, at the time of admission.
• Actively communicate with clients and customers through phone and email correspondence.
• Ensure quality oversight and improvement services.

Requirements and Education:

• Requires a Master’s degree; related experience preferred.
• RTF Clinician requires two years of related experience.
• Professional licensure required: (LMSW with 3 years experience, LMHC-D, LMFT-D, LCAT, LCSW)
• Valid NYS driver's license required
• Must pass physical.
• Must be able to complete TCI training.
