As a Case Manager, Regional Permanency Resource Center you will provide supportive services and case management services to adoptive, guardianship and kinship families so they are able to make informed decisions with regards to services and supports that best meet their needs. You will report to the Case Manager Supervisor. You will work in a hybrid environment with some travel to client's homes in the community.

Responsibilities:

• Function as a systems navigator to families who have adopted or who have children in guardianship or kinship care through the provision of case management.
• Meet with families in their home to assess needs
  • Assess needs, conduct service planning, service plan implementation, service coordination, monitoring and follow-up, reassessment and review with the family via in-home or community monthly visits. Services include but are not limited to information and referral services; referral to legal services; attendance at family court dates; transportation; housing assistance; childcare; basic family needs (vital needs in emergencies and with expenses that are unforeseen); educational advocacy services; and facilitating appointments with social services, medical health providers, and mental health services.
• Respond to the request for in home support referrals for resources and services
• Provide crisis intervention supports when needed
• Work with family to access needed respite services
• Conduct follow up contacts with families at prescribed intervals
• Participate in family engagement activities
• Co-lead support groups
• Participate in training programs offered by the program as assigned
• Use lived experience to empathize and provide hope. Preference for someone with experience with kinship, adoption or guardianship as a youth or in a parenting role.
• Maintain records including required data collection information according to the program standards and expectations. Case Managers must maintain Evolv record with progress notes and all pertinent documentation, a discharge summary and post-case closure documentation.
• Participate as a trainer in programs in the community, to increase understanding of the needs of families who have adopted or have children in guardianship or kinship care
• Complete risk assessments as required by Northern Rivers Quality Management Department

Requirements and Education:

• Bachelor’s level degree in social work, psychology, or human services, or a bachelor’s level degree in any field with at least two years of case management experience; or an associate’s level degree in psychology or human services with at least two years of case management experience.
• Must be able to communicate effectively and develop positive working relationships

As a Licensed Clinical Community Service Providers, you will provide Children and Family Treatment Support Services (CFTSS). These mental health services available with New York State Children's Medicaid program, give children (up to age 21) and their families the power to improve their health, well-being and quality of life. This program is intended to offer opportunities to better meet the behavioral health needs of children early on and to prevent the onset or progression of behavioral health conditions. Northern Rivers Family of Services seeks a leader for the Youth ACT Crisis Response program in Schenectady, NY. Seeking an experienced professional who will develop and supervise a multi-disciplinary team that provides services to youth and families with the highest level of need in the community. The over-arching goals of the program are to prevent out-of-home placement, in-patient hospitalization, and involvement with the child welfare system by expanding services to youth ages 10 to 21 with significant psychiatric needs, who are at risk of institutional level of care, and who require intensive interventions to support the child and family’s complex needs.

The Director will also have the opportunity to develop their team’s skills to maximize their impact on the individuals and communities the Youth ACT program serves while also providing clinical and administrative oversight to the program in collaboration with the NYS OMH and NRFS leadership.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Interface with county mental health leadership to coordinate admissions to Youth ACT and successful discharges to lower levels of care
• Coordinate with prescribers, licensed mental health professionals, and team members with specialized expertise to provide youth and families with comprehensive care
• Facilitate team building, staff training, and staff supervision to develop a cohesive, high performing team
• Collaborate with agency leadership to develop the Youth ACT program as a best-practice model in New York State
• Provide outreach and education to community providers about Youth Assertive Community Treatment (ACT) including the program philosophy, outcomes, and best practices
• Develop and maintain collaborative relationships with other agencies, schools, hospitals, primary care providers, housing authorities, and law enforcement agencies to assist in meeting the needs of the youth and families served

As a Clinician for Mobile Crisis, you will be providing crisis risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), crisis stabilization and interventions, de-escalation techniques and follow-up services to the identified client experiencing emotional, behavioral, and psychiatric crises. These interventions, provided by telephone and on-site mobile visits, will be implemented using a strength-based, trauma-informed, systems-oriented perspective and will include the development of crisis prevention and safety plans; follow-up to evaluate the effectiveness of plans; coordination of services with existing treatment providers; and linkage to additional community-based therapeutic resources and supports. You will directly report to the Clinical Site Supervisors with Director and Executive Program Director guidance and support. Responsibilities:

• Provide rapid response to adults, children, adolescents, and families, who are in crisis, experiencing an emotional or behavioral disturbance by phone and on-site within the community, and as assigned, participate in acute care emergency services rotation system designed to facilitate and strengthen community-based crisis intervention services. This includes rotation of schedule to provide coverage during holidays and weekends.
• Conduct risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), mental status examinations, and substance abuse screening.
• Analyze information to identify action plan, which includes crisis stabilization plan, safety, and crisis prevention plans, and plan for follow-up. Consult with team and supervisors.
• Coordinate with community resources for collateral input and follow-up care, including referrals for mental health treatment and substance abuse assessment and treatment.
• Provide short-term crisis intervention and stabilization services to adults, children, adolescents, and their families in the home or community (wherever the individual is experiencing a crisis).

Requirements and Education:

• Looking for applicants with their LMSW, LMHC, LCSW, LCSW-R, MSW-LP or MHC-LP licensure, must be license eligible and willing to apply for a limited permit in discipline upon hire

St. Lawrence has a unique opportunity for a seasonal caretaker at the University owned property on Upper Saranac Lake (NY). “Canaras” is a private camp with more than two thousand feet of scenic shoreline and several buildings including a main lodge, dining hall, guest cabins, boat houses, docks, tennis court, and conference center. It is operated as a seasonal destination for St. Lawrence University functions and guest accommodations for alumni, faculty, and staff. Our primary operations run from May – August with additional work expectations in April, September, and October. The Caretaker will be required to maintain a presence on-site and be on call during the occupied season.

The Caretaker is responsible for the maintenance and upkeep of all building interiors and exteriors. Responsibilities will include but are not limited to the security of all gates and buildings, door hardware, general carpentry, plumbing, electrical, HVAC, painting, maintaining fire safety devices and record keeping, daily water tests per NYS Board of Health. Sweeping decks, ramps and stairs and clearing roofs of debris.

Responsibilities will further include maintaining grounds to include but not limited to maintaining walkways, trails, roads. Removal of fallen branches, trees, cutting tree limbs and maintaining the canopy, chipping debris, cutting wood, mowing, weed eating and snow removal. The caretaker will also be responsible for training Canaras staff in these activities and to be there during the opening and closing of camp with Canaras staff. The caretaker will be responsible for being on site for all external and internal guests who stay on site during the pre and post season.

Maintain the operating budget and will be heavily involved in project planning and the capital budgeting process. Must maintain all University equipment, tools, and general stores. Additional responsibilities will include maintaining annual physical inventory of all tools, equipment, building furnishings, set-up, and tear down of events.

Under the general direction of the Associate Vice President and Chief Facilities Officer, the Caretaker must be able to interact positively with the Camp Director, Cook, and student workers along with contractors and other University employees.

The Caretaker will perform carpentry duties as assigned by the CFO or Capital Projects Manager.

Salary $37,000 - $42,000.
