POSITION OVERVIEW
POSITION SUMMARY
As a Transition Coordinator, you will coordinate transition services and instruction to enable students to reach their fullest potential and be able to live, learn and earn in their community as an adult. You will report to the Program Manager and will work onsite.
Responsibilities:
- Work with NHS staff, Clinical staff, Parents/guardians, outside service providers and identified students to:
- Design a realistic discharge plan that is guided by each youth’s wishes, hopes and dreams
- Coordinate your client driven discharge planning process with local OCFS, OMH, and NYSED representatives that supports a client-driven plan which integrates housing, health and medical care, education and employment needs.
- Coordinate a set of vocational and transitional related activities including instruction, community experiences, employment and volunteer experiences, and the acquisition of daily living skills.
- Examine/identify post secondary vocational training options
- Respectfully honor each youth’s feelings about past and current relationships and ensure that when needed individuals will have the opportunity to work on issues of grief, loss and enduring relationships
- Complete a Permanency Pact and identify specific people who agree to offer life long supports (either related or not)
- Develop a Circle of Supportand identify specific people who agree to advocate for and support the student during the transition process
- Promote active involvement of identified students in planning and implementing the transition component of their IEP.
- Coordinate services to ensure that each identified student has an accurate Work Skills Employability Profile and earns a Regents Certificate of Work Readiness upon graduation from the Neil Hellman School
- Coordinate transition portfolios for each identified student.
- Provide 6 months of follow up and maintain post-NHS discharge statistics for each identified student.
- Attend and participate in all treatment reviews, case reviews and CSE meetings for identified student
- Complete all documentation in a thorough and timely manner
Requirements and Education:
- Bachelor's Degree and at least 3 years of experience working with individuals who demonstrate serious emotional and behavioral challenges.

Are you looking to make a difference in the lives of youth and families struggling with behavioral health issues? Do you want to develop and supervise your own team and mentor your staff to improve their clinical skills? As the Team Lead of Home Based Crisis Intervention (HBCI), you will provide clinical supervision, support, and mentorship to your team to help them change lives. The HBCI team provides short-term (up to 6 weeks), intensive intervention to youth and families who are in crisis. We work with parents and caregivers to help them help their children. The short-term format means the work is fast-paced and outcome oriented, which means you and your team will produce meaningful outcomes in a short period of time. You will report to the Chief of Crisis Services and work on-site with travel required.
Responsibilities:
- Provide clinical supervision to the team and be responsible for allocation of staff time; provide training and ensure all team members are kept informed of issues.
- Monitor caseload size and coordinate program intakes and discharges.
- Create rotating call schedule for after-hours / weekend coverage by team members.
- Provide education and outreach to the community about HBCI at Northern Rivers; philosophy, program outcomes, theoretical background etc.
- Develop collaborative relationships with other agencies, schools, hospitals, primary care providers, and local government agencies to help meet the needs of this population so they can achieve and maintain their independence.
- Assist primary clinicians to conduct needs assessments, safety planning and wellness management planning with participants. Connect participants and families with the services and support they need. Provide psychoeducation regarding mental health diagnosis and treatment. Develop treatment using a shared decision-making process with clients.
- Assist primary clinicians to respond to on-call needs of families in crisis and deliver crisis intervention services in home-based environment with flexible scheduling cognizant of family needs, with 24/7 availability nights, weekends and holidays.
- Promote safety of the child, family, and community in the delivery of crisis intervention services.
- Assist primary clinicians to conduct comprehensive assessment of family strengths, values, skills, problems, need and barriers to goal attainment while developing a positive working relationship with the families.
- Collaborate and advocate with formal and information community resources and systems that impact family while teaching family members to advocate for themselves and prepare them for transition and service closure.
Requirements and Education:
- Requires a Master’s degree and at least one year of progressively responsible related experience. Experience in a home-based and/or crisis intervention services program. Candidates with training in the Homebuilder model of crisis intervention are preferred.
- Must be a licensed mental health professional. These include, but are not limited to: Licensed Psychologists, Licensed Clinical/Masters Social Workers, Licensed Marriage and Family Therapists, Licensed Mental Health Counselors, or Licensed Creative Arts Therapist.
- Must have a valid driver’s license.

As a Program Coordinator, you will be responsible for the administrative and programmatic oversight of a Foster Care team. You will oversee staff, the foster families associated with the office and maintain agency resource relationships within the counties the foster care program supports. You will report to the Director of Foster Care and will work onsite.
Responsibilities:
- Supervise multistage foster care treatment teams in areas of intake and ongoing treatment planning for foster youth and foster families.
- Ensure the diligent efforts made to reach permanency of foster children in placement.
- Coordinate the work of a multidisciplinary team comprised of permanency care managers, and clinical and nursing departments to fulfill these goals of permanency, safety, and wellbeing for our children in care.

- Work with the Local Department of Social Services to fulfill contracted services.
- Develop and maintain relationships with outside agencies for resources to program.

- Ensure case records are maintained to the standards of NY OCFS regulations, COA standards as well as agency standards.
- Collaborate with Northern Rivers service departments including finance and human resources.

Job Summary | The Department of Physics, Applied Physics, and Astronomy at Rensselaer Polytechnic Institute in Troy, NY invites applications for a tenure-track Assistant Professor position in the field of Quantum Computing, Quantum Information, and Quantum Technology, with focus on the fundamental aspects or applications of quantum computation, scalability, and error correction on current and future quantum computing devices. Research areas can include but not limited to quantum entanglement, decoherence, and noise in quantum networks, with applications to quantum cryptography, quantum encryption, and secure quantum communications in noisy environments. Possible related areas would also include studying and exploiting the fundamental connections between Quantum Monte Carlo and Quantum Computing, simulating quantum field theory and many-body theory with a quantum computer (in particular, simulating systems with a “sign” problem), quantum communication networks, exotic quantum dynamics, quantum sensing protocols, exotic quantum phases of matter, and theory on the nature of information at the quantum level. With the anticipated of arrival of the IBM Quantum System One on our campus, this hire will have access to explore and implement some of these challenging problems. This hire would be part of a bold effort at RPI to strengthen and extend our current faculty cluster in Quantum Information, and would interact with our current faculty on quantum optics and photon entanglement, quantum field-theory, quantum fluctuations in networks, as well as with researchers from Computer Science, ECSE, Future of Computing Institute, and with the future hires of the recently announced Quantum Constellation. The position also represents a response to federal priorities expressed in the National Quantum Initiative Act. The successful candidate will establish/maintain and externally funded research program, participate in collaborative, multi-investigator research and research training initiatives, and contribute to the high impact teaching program in the Department of Physics, Applied Physics, and Astronomy. As a Clinician for Mobile Crisis, you will be providing crisis risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), crisis stabilization and interventions, de-escalation techniques and follow-up services to the identified client experiencing emotional, behavioral, and psychiatric crises. These interventions, provided by telephone and on-site mobile visits, will be implemented using a strength-based, trauma-informed, systems-oriented perspective and will include the development of crisis prevention and safety plans; follow-up to evaluate the effectiveness of plans; coordination of services with existing treatment providers; and linkage to additional community-based therapeutic resources and supports. You will directly report to the Clinical Site Supervisors with Director and Executive Program Director guidance and support. You will work on-site with travel to the community and individuals' homes.
Responsibilities:
- Provide rapid response to adults, children, adolescents, and families, who are in crisis, experiencing an emotional or behavioral disturbance by phone and on-site within the community, and as assigned, participate in acute care emergency services rotation system designed to facilitate and strengthen community-based crisis intervention services. This includes rotation of schedule to provide coverage during holidays and weekends.
- Conduct risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), mental status examinations, and substance abuse screening.
- Analyze information to identify action plan, which includes crisis stabilization plan, safety, and crisis prevention plans, and plan for follow-up. Consult with team and supervisors.
- Coordinate with community resources for collateral input and follow-up care, including referrals for mental health treatment and substance abuse assessment and treatment.
- Provide short-term crisis intervention and stabilization services to adults, children, adolescents, and their families in the home or community (wherever the individual is experiencing a crisis).
Shift/Schedule:
- Fulltime; 5 days a week 8 hour shifts or 4 days a week 10 hour shifts
- Cross shifts: 10am-6pm, 11am-7pm, or 12pm-8pm
- Part time hours available
- Weekend and holiday expectations
- When scheduled for a weekend shift your schedule will be 5, 8 hour shifts for that week
Requirements and Education:

As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.
Responsibilities:
- Level I - Provides care management services to children with low and medium acuities. The Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in person with the member.
- Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.
- Engage children, youth, and families with outreach and enrollment activities for successful enrollment into a children's health home
- Gather and complete assessments and enter documentation that confirms eligibility and acuity, identifying both strengths and areas of need
- Work with children, youth, families, and service providers to create a comprehensive Plan of Care
- Identify and engage additional providers (e.g. natural supports, parent advocates, primary care physicians) with youth and families for the execution and continuation of the Plan of Care

POSITION OVERVIEW
As a Skillbuilder, you will provide targeted skill training to help with independent living and social skills. Goals that the Skill Builder will work on could be Career Planning, Self-Care, Daily Living, Housing, and Money Management, including establishing and maintaining interpersonal relationships, how to accept no as an answer, handling conflict with peers and authority figures, and using their coping skills when feeling dysregulated. You will report to the Program Coordinator and you will work onsite.
Responsibilities:
- At a minimum, 50% of the work week is spent face-to-face with clients to provide services designated in the Treatment Plan.
- Provide services based on the youth's needs and preferences as identified in the Treatment Plan.
- Work with treatment team members to identify goals and objectives of treatment, including youth, families, school, and counselors.
- Complete all required documentation and submit it to the Supervisor within the required timeframes.
- Ability to work with diverse consumers who have mental health challenges and may be developmentally delayed. Understand their independent living needs to work with the case manager to find resources in the community for educational and vocational skills and programming for discharge.
Requirements and Education:
- High School Diploma/GED required
- At least 2 years of experience working with children
- Valid driver's license required

Directly responsible for providing non-clinical services to individuals with mental health needs who reside in Saratoga County. This is an in person role, in Saratoga Springs, New York.
Essential duties and responsibilities include the following:
- Facilitate the Peer Run Social Club.
- Provide coverage for Saratoga County Warm line as needed.
- Provide training and teach skills needed to navigate mental health services in Saratoga County to identified clients.
- Promotes socialization, recovery, self-sufficiency, development of natural supports as well as coping skills and goal setting for those individuals with mental health issues.
- Act as an advocate by assisting individuals with housing issues, medical concerns, therapy or medication related problems, employment goals, and basic budgeting skills.
- Assist with benefits counseling.
- Engage participants in shared decision making around services and recovery planning.
- Function as part of a participant’s treatment to ensure integrated, collaborative care.
- Assist participants with the identification of activities that promote a sense of prosocial community engagement.
- Participate in regularly scheduled supervisions.
- Complete daily case notes.
- Complete all required agency trainings as well as trainings and certifications set forth by the New York State Office of Mental Health.
- Must possess strong interpersonal boundaries.
- High School or GED required.
- At least two years’ experience with the Mental Health system in Saratoga County.
- Per regulatory requirements, must have life experience with mental health and/or recovery from
- substance abuse.
- Peer Empowerment Credential required for Adult HCBS.
- Must have a valid NYS Driver’s license. Ability to operate agency vehicles is required.

Job Summary | The Lally School of Management at Rensselaer Polytechnic Institute seeks applications for a tenure-track (or tenured) faculty position at the Associate Professor level in Business Analytics/Supply Chain Management starting Fall 2024. Candidates must have a PhD degree from an accredited university in Analytics, Operations Research, Operations Management, Supply Chain Management, or a closely related field, along with demonstrated evidence to produce original and innovative scholarly work with publications in premier business journals. The successful candidate will join the School either as a tenure-track assistant professor, or as an associate professor, based on teaching and research experience and success. The successful candidate will have a demonstrated potential and commitment to effective teaching in our graduate and undergraduate degree programs in supply chain and business analytics, as well as provide service to the School of Management and to Rensselaer. The position requires excellence in research in the area of supply chain and analytics, effective written communication skills, and strong technical skills in areas such as analytical modeling, data management, machine learning and computation. Demonstrated interest in emerging technologies including artificial intelligence and Blockchain and in topics relevant to global shifts in supply chains would be a plus. The successful candidate will have the opportunity to engage with and develop research initiatives aligned both within the Lally School and with the new strategic research initiatives at Rensselaer. Job Summary | The Department of Biological Sciences at Rensselaer Polytechnic Institute in Troy, NY invites applications for tenure-track/tenured faculty positions at the Assistant, Associate, or full Professor levels. We seek passionate and innovative individuals with demonstrated potential to establish a strong externally funded research program and contribute to the teaching mission of the department. Successful candidates will complement the department research strengths in cellular neuroscience, circadian biology, molecular motors & cytoskeleton dynamics, muscle physiology & disease, neurodegeneration, protein design, folding, structure & functional dynamics, protein misfolding in human disease, microbiology & host-pathogen interactions, biophysics of extreme environments, and freshwater ecology & climate change. The Institute for Data Exploration and Applications (IDEA) enables an opportunity to partner with faculty that are at the forefront of biological data analytics, enabling the modeling and simulation power to advance the quantitative and systems biology approaches that are widely employed in the department. IDEA and the Center for Computational Innovation provide collaborative opportunities to use the fastest supercomputer found at a private university. School Based Clinician
The School Based Clinician is responsible for providing clinical services to youth and their families according to program and regulatory guidelines, ensuring that all services are strengths-based, individualized, and comprehensive according to need.
RESPONSIBILITIES:
- Provide clinical services to youth and their families as assigned.
- Participate in the integrated service planning process for all youths enrolled.
- Ensure all service plans are strengths-based, individualized, and comprehensive according to need.
- Monitor the effectiveness of integrated service plans and review.
- Ensure all service are coordinated with other service providers.
- Access supports in the School or in the community as indicated.
- Assist families as needed in accessing assessments and recommendations for psychotropic medication.
EDUCATION and/or EXPERIENCE REQUIREMENTS:
- Requires a Masters degree, MSW or equivalent.
- Experience, training, or education commensurate with working with children with serious and persistent emotional disturbance is desired.
- Experience with working in a school environment is preferred.
- Must be licensed prior to starting, or have an active limited permit. LMSW, LCSW, or LCSW-R preferred.
- Required to pass licensing exam within six months of employment if working with a limited permit.
- Drivers license is required.

Job Summary | Endowed Constellation Faculty in Future Chips at Rensselaer Polytechnic Institute We are especially interested in candidates who have a demonstrated ability to perform high impact research in semiconductor technology to address the next generation challenges in computing. Areas of focus are rooted in fundamental science and engineering that drive advances in microelectronics, such as heterogeneous integration, advanced logic, advanced packaging, and power electronics. Physics of materials is also important to making these advances and is, therefore, an area of interest. The ideal candidates are expected to develop an internationally recognized, externally funded, scientific program in chip technology for the future of computing. The Future Chips Constellation will be a key stakeholder in the Center for Materials, Devices, and Integrated Systems (cMDIS) which houses the Microscale and Nanoscale Cleanroom and the Nanoscale Characterization Core.
