Clinician, Positive Youth Program
As a Clinician in the Positive Youth Progression program, you will maintain a caseload of eight to ten youth and meet with each weekly. Positive Youth Progression provides six to nine months of home and community-based prevention services to youth who are at risk of being removed from their homes. The goals of the program are to prevent out-of-home placement for troubled youth and to avoid the juvenile justice system. You will report to the Program Coordinator and you will work with a partner (Clinical Case Manager) in a combination of on-site and remote with travel to client's homes is required (travel reimbursement offered).
Responsibilities:
- Complete assessment to identify the strengths and needs of the youth, caregiver and family.
- Develop treatment plans for the youth and families to help identify and meet their goals.
- Provide home and community-based clinical treatment coupled with supportive and skill based wrap around casework interventions based on assessment, planning and treatment with youth and their families
- A trauma-informed approach to service delivery and working with the clinical case manager to conduct family sessions
- Communicate with the youth’s family to monitor progress and support the family’s effort to keep the youth safe
- Ensure provisions of regular treatment plan meetings and services plan reviews to assess on going progress and identify any needed modification to treatment planning
- Coordinate services with service providers and collaterals involved with the youth and their family
- Complete paperwork within agency, count, and state guidelines.
- You will work Monday-Friday with flexible schedule that includes evenings and a rotating on-call schedule.
Your Experience and Qualification:
- Masters degree in social work, mental health counseling, or equivalent from an accredited graduate school program
- 0-2 years post-masters experience
- Experience working with at-risk youth and their families preferred
- Home and community based experience is a plus

As a Community Service Provider, you will provide home and community-based services to help children with emotional and mental health challenges. You will travel locally to work with children and families in their communities providing a core of rehabilitative services to ensure success, including non-licensed counseling and support for youth and family, skill building, peer support, caregiver support and advocacy, pre-vocational services and respite. You will report to a Community Service Provider Supervisor, and work onsite within the community, as travel is required and documentation may be completed remotely.
Responsibilities:
- Provide services within a Family-driven Care framework, including respecting family choice and voice, believing in each family’s potential to learn and grow, and utilizing strategies that empower families
- Work with family members to identify goals and objectives of treatment
- Provide services based on the needs as identified in the Treatment Plan
- Coordinate with the child and caregiver on the scheduling and provision of services
- Participate in relevant treatment meetings and attend all required trainings
- Complete all documentation
Requirements and Education:
- Bachelor's degree in a related field and at least two years of related experience OR
- Master's degree in Social Work, counseling, psych, or a related human service and at least one year of related experience
- Valid NYS Driver's License required
