Responsibilities

Responsibilities:

• Assess and evaluate the health care needs of patients and families with consideration regarding physiological, psychological, social and environmental factors.
• Identify complete, accurate, and logical OASIS data and appropriate sequencing of diagnoses.
• Develop a care plan based on patient needs, physician orders, nursing assessment parameters and identified goals that are respectful of patient, family, community, and agency resources.
• Implement the care plan and utilize nursing judgements to modify plan as needed.
• Evaluate for and promote a safe environment for the patient and complies with JCAHO National Patient Safety Goals.
• Meet requirements for OASIS lock dates, re-certifications, and discharge timeline.
• Communicate with other health care professionals both internal and external to the organization in a manner that assures care plan coordination as well as continuity of care.

Qualifications

Qualifications:

• Current NYS registration to practice professional nursing
• 1 year of general nursing experience with current home health, med/surg, or critical care experience
• Driver’s license, insurance, and transportation for work as required

Education:

• Associate’s degree in Nursing from an Accredited College/University

Job Type:Full-time

Salary:$114,000 to $118,000 per year
