Description
As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.
Responsibilities:
- Level I - Provides care management services to children with low and medium acuities. Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in-person with the member.
- Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.
- Engage children, youth, and families with outreach and enrollment activities for successful enrollment into a children's health home
- Gather and complete assessments and enter documentation that confirms eligibility and acuity, identifying both strengths and areas of need
- Work w/children, youth, families, and service providers to create a comprehensive Plan of Care
- Identify and engage additional providers (e.g. natural supports, parent advocates, primary care physicians) with youth and families for the execution and continuation of the Plan of Care
- Convene and conduct Interdisciplinary team meetings to review Plan of Care; identifying progress, continued needs, and confirming ongoing eligibility
- Document progress and arrange for transitions between systems, placements, and higher levels of care following changes in eligibility and acuity.
- Use NY State and Health Home information systems to input required Health Home documentation
- Comply with Medicaid and program billing regulations
- Complete all required and recommended trainings
Requirements and Education:
- Level 1 - Requires an Associate's degree in Social Work or a related field, with 2 to 5 years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
- Level 2 - Requires a Bachelor of Arts or Science degree, and two years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
- You have a valid driver's license and automobile that is insured following New York State Requirements

Job Summary | The Sr. Major Gift Officer (MGO) will serve as the Senior Regional Director of Development for the Southwest Region of the United States and will be responsible for developing a regular cadence of strategic opportunities focused on engaging existing and prospective major donors with the end goal of qualifying, soliciting, and closing major gifts. This role will serve as the primary strategist and staff person for the Texas Triangle Executive Council. As a Lifeguard, you will provide rescue and lifesaving services, teach water safety and swim instruction to children and adolescents in program during the summer months, and help maintain our pools for optimal health and safety. You will report to the Director of Therapeutic Recreation and you will work onsite.

- Monitor and maintain order in the swimming pool area to prevent accidents
- Rescue swimmers in danger of drowning, administering first aid

As a Foster Clinician, you will provide clinical services to children in foster care and support foster families, and families within the foster care system. The Therapeutic Foster Family Program provides foster and pre-adoptive foster homes for abused and neglected children from birth through age 21. - Provide assessment, treatment planning and general counseling for children in foster care.

As a Greenhouse Coordinator, you will development and implementation of the botanical classroom instructional plans for students. You will report to the Transition Learning Coordinator and you will work onsite.
Responsibilities:

- Follow school and agency procedures and policies for reporting incidents. This includes behavior incident reports (IR) and incidents such as child/sexual abuse, substance abuse, harassment, bullying, and violence.
- Establish relationships with parents and family members to create a partnership around student treatment, student behavior, and academic progress.
- Participate in classroom grade level, and behavior/treatment meetings as well as professional training and development sessions (e.g. TCI, CPR, emergency procedures and other agency requirements.
- Follow all Agency policies and procedures, along with state and federal regulations pertaining to our school and students.
- Facilitate partnership between the greenhouse activities and gardens through home and school education programs by coordinating community workshops, recruiting home garden volunteers.
- Work with other teachers, clinicians, administrators, medical staff and other school staff to provide integrated support, assistance, and treatment to students.

As a Speech Pathologist, you will provide Speech Therapy services to the school and the amazing students that we have. You will report to the clinical Supervisor and you will work onsite.
