Licensed Community Clinician
Remote/Hybrid Role

As a Licensed Clinical Community Service Providers, you will provide Children and Family Treatment Support Services (CFTSS). These mental health services available with New York State Children's Medicaid program, give children (up to age 21) and their families the power to improve their health, well-being and quality of life. This program is intended to offer opportunities to better meet the behavioral health needs of children early on and to prevent the onset or progression of behavioral health conditions. These services strengthen children and their families, and help them to make informed decisions about their care and are provided either at home or in the community. You will report to the Clinical Supervisor and you will work remote/hybrid.
Responsibilities
- Provide Home and Community-based Assessments, counseling, and treatment planning to children and their care givers.
- In Person, in the home, community-based, in office and telehealth to meet the needs of our clients.
- Complete an assessment of needs and recommendations for further medically necessary services, such as rehabilitation services. Services are delivered in a trauma informed, culturally and linguistically competent manner.
- Work with family members, advocates and other salient parties to identify goals of treatment. Provide observations and information to collateral contacts for the purpose of the progress and tracking of the Treatment Plan goals.
- Complete all required documentation for submission to the Supervisor within the required timeframes.
Requirements and Education
- Masters degree in Social Work, Counseling, Psychology or related field.
- 1 year of related experience.
- NYS Professional Licensure/Limited Permit - LMFT, LMSW, LMHC, LCSW, and LCAT.
- NYS drivers license with a reliable vehicle, as work requires up to 50% community travel/client home visits.

Clinician, Intensive Aftercare Prevention Program

As a Clinician in the Intensive Aftercare Prevention program, you will provide home-based clinical services to children and their families to help preserve or reunite them following Northeast Parent & Child Society's stated purpose and mission of protecting children, preserving families, and strengthen communities. The Intensive Aftercare Prevention Program (IAPP) is a home and community-based program assisting families who have children in foster care or children who are at risk of being placed in foster care. You will report to the Program Coordinator and you will work remotely with travel to client homes required.
Responsibilities:
- Provide home and community-based assessment, planning and treatment with individuals, groups and families with those enrolled in our Family Preservation services (the level of intensity / number of direct services hours per family is dependent upon the services in which one is employed)
- Complete necessary all records and client information following agency, county, and state policy and funding requirements
- Provide liaison contact with collateral resources such as referring agencies, schools, family court, and other agency personnel serving on behalf of a child and family as it relates to their treatment
- Responsible for the clinical decisions when assigned and paired to work as a team with a Bachelors Clinical Case Manager.
- Ensure provisions of regular treatment plan meetings and services plan reviews.
Requirements and Education:
- Requires a Masters degree in Social Work, Mental Health Counseling or related field
- Minimum of one year of post-graduate experience.
- License Preferred: LMSW, LCSW, LMHC, or LMFT
- Must have a valid drivers license.

Description
As a Team Lead of Home Based Crisis Intervention (HBCI), you will providing short-term, home-based crisis intervention services to youth and their families who are at risk of institutional level of care, and who require intensive interventions to adequately support the child and family’s complex needs. You will report to the Chief of Crisis Services and work on-site with travel required.
Responsibilities:
- Provide clinical supervision to the team and be responsible for allocation of staff time; provide training and ensure all team members are kept informed of issues.
- Monitor caseload size and coordinate program intakes and discharges.
- Create rotating call schedule for after-hours / weekend coverage by team members.
- Provide education and outreach to the community about HBCI at Northern Rivers; philosophy, program outcomes, theoretical background etc.
- Develop collaborative relationships with other agencies, schools, hospitals, primary care providers, and local government agencies to help meet the needs of this population so they can achieve and maintain their independence.
- Assist primary clinicians to conduct needs assessments, safety planning and wellness management planning with participants. Connect participants and families with the services and support they need. Provide psychoeducation regarding mental health diagnosis and treatment. Develop treatment using a shared decision-making process with clients.
- Assist primary clinicians to respond to on-call needs of families in crisis and deliver crisis intervention services in home-based environment with flexible scheduling cognizant of family needs, with 24/7 availability nights, weekends and holidays.
- Promote safety of the child, family, and community in the delivery of crisis intervention services.
- Assist primary clinicians to conduct comprehensive assessment of family strengths, values, skills, problems, need and barriers to goal attainment while developing a positive working relationship with the families.
- Collaborate and advocate with formal and information community resources and systems that impact family while teaching family members to advocate for themselves and prepare them for transition and service closure.
Requirements and Education:
- Requires a Master’s degree and at least one year of progressively responsible related experience. Experience in a home-based and/or crisis intervention services program. Candidates with training in the Homebuilder model of crisis intervention are preferred.
- Must be a licensed mental health professional. These include, but are not limited to: Licensed Psychologists, Licensed Clinical/Masters Social Workers, Licensed Marriage and Family Therapists, Licensed Mental Health Counselors, or Licensed Creative Arts Therapist.

As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. - Level I - Provides care management services to children with low and medium acuities. Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in-person with the member.

As a Speech Pathologist, you will provide Speech Therapy services to the school and the amazing students that we have. You will report to the clinical Supervisor and you will work onsite.
Responsibilities:
- Provide individual and group speech and language therapy in person .
- Provide diagnostic speech and language evaluations and screenings.
- Write, maintain and chart records of children approved for speech and language services.
- Confer with classroom teachers in preparing daily speech and language activities for children in the Special Education Program.
- Complete initial and ongoing evaluations.
- Develop and implement IEP goals and objectives.
- Provide direct play-based child oriented therapy.
- Help supervise individual children and groups (progress monitoring)
- Complete reports, therapy notes, progress monitoring, and logs according to applicable time requirements.
- Supervise the Teacher of the Speech and Hearing Handicapped (TSHH) staff working under their direction, and complete supervision documentation.
Requirements and Education:
- Requires at least one year experience working with students pre-K-12 with developmental disabilities. Knowledge of diagnostic, assessments and report writing. Familiarity with therapeutic diagnostics, assessments and report writing. Familiarity with therapeutic approaches dealing with language, articulation and oral-motor skills.
- Masters Degree in Speech Pathology.
- NYS Certification, license.
- ASHA Certification preferred.

As a Foster Clinician, you will provide clinical services to children in foster care and support foster families, and families within the foster care system. The Therapeutic Foster Family Program provides foster and pre-adoptive foster homes for abused and neglected children from birth through age 21. Each foster parent receives intensive training and ongoing professional support to promote a positive, successful experience. You will report to the Clinical Supervisor and you will work onsite.
Responsibilities:
- Provide assessment, treatment planning and general counseling for children in foster care.

As a Clinician in the Intensive Aftercare Prevention program (IAPP), you will provide six to nine months of intensive services to families whose children are either at risk of being removed from their homes or are already in placement. As a Coordinator of Short-term Services, you will be the face of Parsons at Schenectady Department of Children and Families and will collaborate with this department, while supervising Family Support Workers to reach positive outcomes for referred families. The Parsons Short-term Services Program is an intensive case-management service designed to rapidly resolve the concrete and referral needs of families entering the Child Welfare System. By resolving these issues within 90 days of admission, families are at a reduced risk for continued involvement in the child welfare system. As a Greenhouse Coordinator, you will development and implementation of the botanical classroom instructional plans for students. You will report to the Transition Learning Coordinator and you will work onsite.
Responsibilities:
- Provide subject area instruction (Green house and plan based botanical) that provides students with appropriate level lessons and age experiences.
- Develop and implement instructional materials to provide individual and small group instruction to students of various ability levels grades 8 -12.

- Represent the greenhouse and gardens at relevant conferences, community events andnetworking opportunities. Conduct workshops, tastings, classroom activities and garden clubs.
- Manage and coordinate the safety and well-being of students through the use of established classroom and schoolwide processes and procedures; supervise classroom staff in implementing these with their work with students throughout the building.
- Follow school and agency procedures and policies for reporting incidents. This includes behavior incident reports (IR) and incidents such as child/sexual abuse, substance abuse, harassment, bullying, and violence.
- Establish relationships with parents and family members to create a partnership around student treatment, student behavior, and academic progress.
- Participate in classroom grade level, and behavior/treatment meetings as well as professional training and development sessions (e.g. TCI, CPR, emergency procedures and other agency requirements.
- Follow all Agency policies and procedures, along with state and federal regulations pertaining to our school and students.
- Facilitate partnership between the greenhouse activities and gardens through home and school education programs by coordinating community workshops, recruiting home garden volunteers.
- Work with other teachers, clinicians, administrators, medical staff and other school staff to provide integrated support, assistance, and treatment to students.
- Assist with supervision of school-to-work program activities as needed.
