ArchCare Senior Life (PACE) is a long-term Medicaid/Medicare plan designed specifically for seniors who need nursing home level care but want to stay at home. With Senior Life, patients have a dedicated Care Team to coordinate everything, so they are able to live comfortably where they are, knowing they’ll get the coordinated care they need. PACE provides and coordinates care at our Wellness Centers, in patient’s homes and in other settings.

Responsibilities
Responsibilities:
- Develops and maintains a therapeutic relationship with the participant to optimize participant functioning by providing quality services in conjunction with the interdisciplinary team.
- Develops an individualized life plan with the participant identifying his or her goals and implementing interventions to assist with achieving their goals.
- Monitors effectiveness and outcomes regularly and keeps the team informed as to participant progress and level of need. Remains alert to pertinent input from other team members, participants, and family members/caregivers.
- Documents changes of a participant’s condition in the participant’s medical record consistent with documentation policies established by the medical director.
- Manages social work planning for a caseload of participants. Maintains productivity sufficient to meet program goals.
- Assists the interdisciplinary team members including the physician, as appropriate, in understanding the significant social and emotional factors related to the member’s health problems.

Responsibilities
- Creates referral generating opportunities by identifying, pitching, and managing ongoing relationships with external accounts, internal system business lines, and other resources to directly facilitate enrollment growth.
- Develops differentiated and customized value propositions that effectively position ArchCare as a preferred business partner to all stakeholder groups relevant to influencing plan growth.
- Performs regular oversight of account relationships, providing responsive communications, reoccurring reporting, troubleshooting assistance, and any other customer service-oriented functions to maintain and improve partnerships.
- Recommends product and/or service enhancements and improvements necessary to maintain a competitive stance within the local marketplace.
- Consistently monitors individual, team, and overall plan performance metrics to facilitate informed and data-driven strategic plan development on outreach and engagement tactics to administer for achievement of enrollment goals.
- Monitors and holds accountable all direct and indirect stakeholders involved in the processing and conversion of referrals, with emphasis on vendors/agencies that oversee requisite stages to complete an enrollment and represent risk to bottlenecking productivity.
- Conducts regular market analysis of industry changes, competitor performance, and regulatory policy shifts affecting managed care, home care, and other closely related businesses.
- Works closely and advises marketing, provider relations, intake & enrollment, plan operations, and other relevant departments to optimize enrollment performance, mitigate limitations or barriers to growth, and provide market feedback/intelligence as needed.
- Closely advises on the efficacy and development of marketing and outreach collateral, with direct guidance on consumer preferences and profiles to aid in the engagement and resonance of materials with target audiences.
- Identifies, coordinates, and participates in community outreach activities to promote the ArchCare brand and any of its affiliated programs through events, speaking engagements, sponsorships, or any other public-facing opportunities.
- Meets monthly enrollment goals while demonstrating empirically influenced analysis of performance is continuously incorporated into driving the strategies employed for the ensuing enrollment period.
- Assists in retention, compliance, educational, or other plan initiatives as requested, performing outreach and/or communication tasks to relevant stakeholders.
- Meets a standardized set of competencies for the specific position description established by the organization and approved by DOH/CMS before working independently.
- Performs other related duties, as required.
