Responsibilities

The LPNis responsible foradministration of all medications andtreatments duringtheir shift, according to the established medical and nursing care plan. Works under the direction of the Head/ChargeNurse toprovideoptimumnursing care to all residents on the unit.

• Continuously evaluates behavior, symptoms,reactions,and progress of residents and discusses findings with Nurse Manager/Charge Nurse.
• Administers and documents all medications according to established policies and procedures, physician's orders, and Department of Health regulations.
• Performs all treatments according to physician’s specific order and established policies and procedures.
• Discusses the need todiscontinueor change a medication of treatment with the Nurse Manager/ Charge Nurse.
• Orders prescribed medications, notifies pharmacy of discontinued medications: and ensures accuracy of medications and dosages when received from pharmacy.
• Coordinates the planning, implementation, and evaluation of resident care services on a 24-hour basis.
• Utilizes knowledge of current trends and practice standards of nursing including geriatric, adult resident, and Assisted Living resident concepts to coordinate staff responsibility to provide quality care.
• Monitors staff compliance with facility policy and procedures, as well as state and federal regulations.
• Audits records to ascertain compliance with facility standards.
• Directs and supervises PCA, Home Health Aide, CNAs, and other team members as necessary.

Salary: $27.9452 - $33.8266 per hour

Responsibilities

Responsibilities:

• Responsible for providing comprehensive primary care, coordination of 24-hour care, medical coverage, and directly provides services to ArchCare participants.
• Provides face-to-face comprehensive History and Physical assessments on new referrals. Prepares face-to-face Quarterly or as needed History and Physical assessments on established participants.

• Provides evaluation and treatment of participants with acute illnesses.
• Participates in the development and revision of the participant's plan of care as a member of the interdisciplinary team. Integrates the primary care treatment plan into the plan of care developed by the interdisciplinary team. Interacts with other team members to meet emergent and acute needs of participants. Participates in discharge planning.
• Documents changes in the participant's condition and details care provided by completing all required documentation.
• Collaborates with nurse, nurse practitioner and other physicians to ensure comprehensive and continuous care.
• Assists in monitoring the medical care of enrolled members who are hospitalized in out-of-network and out-of-service area facilities and assists in their transfer to in-network facilities when medically appropriate.

• Attends and participates in scheduled staff meetings and participant care meetings as requested.

Job Type: Full Time

Salary: $250,000 - 260,000 base plus 35K sign on bonus

Qualifications

Qualifications:

• 1 year of post graduate experience in Internal or Family Medicine
• Board Certified in Internal Medicine or Family Practice

Education:

• MD Degree
• Completion of residency in Internal or Family Medicine

Responsibilities

Responsibilities:

Provide health related tasks under the supervision of a Registered Professional Nurse in a way that is consistent with overall participant safety and accepted standards of participant care.

• Assist the participant with the use of medical equipment and self-administered medication as prescribed.
• Assess, record and report participants' condition as needed including pulse, temperature, BP, signs of discomfort, etc.

• Assist with nutritional and fluid intake, documenting and notifying nurses on dietary preferences.
• Establishing positive rapport with the participants and ensuring confidentiality.
• Provides supervision, companionship and care to individuals with physical and/or cognitive impairment and assists in socialization and recreational activities.
• Knowledge of medical instruments and administration of care, common safety hazards and organizational policies regarding patient care.
• Ability to react calmly and effectively in emergency situations and communicate clearly.

• May perform ambulation and transfer activities under direction.

Job Type: Per Diem

Salary: $17.00 - $18.00 per hour

Qualifications

• Approved NY State HHA Training Program or equivalent
• First Aid Certification required

• Implements administrative goals and policies and interprets objectives of Nursing Services to the nurses on the units.
• Assist in ensuring appropriate staffing is met during shift.

• Rounds the nursing units daily to ascertain the condition and needs of the residents and informs Director of Nursing of all changes and related problems.
• Observes nursing care of the residents to ensure that orders are carried out as directed and treatment is administered in accordance with physician's instructions.
• Assists with orientating newly employed nurses and clinical placement of nursing students.
• Arranges hospital transfer of residents.
• May render nursing care, administer medication and perform treatments.

• Assists in planning, developing, organizing and implementing departmental goals and objectives.
• Ensure appropriate staffing levels are met for each unit by coordinating with Staffing Coordinator
• Exhibits creativity and initiative in pursuing organizational and departmental growth and development.

Salary: $50.29 - $62.87 per hour

• 1 year supervisory experience

Responsibilities:

Under the direction and supervision of an RN or LPN, performs various resident care activities and related non-professional duties for the specific needs and comfort of the aged and those that need our help.

• Assist residents with activities of daily living to include: bathing, dressing, grooming and toileting.
• Transfer residents to and from activities and meals according to their individual service plans.
• Serve meals to residents in the dining room or their apartments.

• Record and report changes in residents' eating habits to supervisor.
• Promote quality services within company, state and federal regulations.
