? Assess & Support Patients Conduct psychosocial assessments, provide emotional support, and help patients cope with their diagnosis, treatment, and prognosis.? Coordinate Care Develop and implement discharge plans, facilitate transitions to post-acute care, and collaborate with an interdisciplinary team to ensure seamless patient care.? Advocate for Patients Assist with cases involving abuse, mental health, substance dependency, guardianship, and other complex social issues.? Utilization Management Monitor patient care progression, resolve obstacles to discharge, and ensure effective resource utilization.? Collaborate & Communicate Work closely with healthcare teams, community agencies, and payers to secure resources and advocate for the best patient outcomes.

? Masters Degree in Social Work (MSW)? Licensed Social Worker (LMSW) in Michigan (or working toward full licensure within three years)? Experience in case management, discharge planning, or healthcare social work (preferred)? Strong communication, problem-solving, and advocacy skills? Ability to work independently while collaborating with physicians, patients, and payers
