Responsibilities

Responsibilities:

• Assesses the environment of the home and community to identify elements which impact negatively on the patient/family ability to function.
• Provides direct short-term counseling services to patient/family or makes appropriate community referrals.
• Teaches coping skills to patients/families to assist them in their adjustment to the life changes associated with illness/disability.

• Participates in conferencing and planning with other disciplines caring for the patient.
• Prepares clinical progress notes and documents in the patients record, as indicated.
• Available for planned social work consultation on patient care with staff, as needed.
• Participates in evaluating overall position performance, goal setting and achievement, and performance improvement plan.

Hudson Valley Credit Union is currently recruiting for 3 (Three) Seasonal Tellers! This program is a full time (40 hour) opportunity that runs over the summer months (5/16, 6/6, or 6/13 - 9/5). Orientation will take place in Wappingers Falls or Middletown, NY. Pay starts at $20/hr.

• Accurately complete all types of account transactions requested by members by following organizational processes and procedures. This includes processing negotiable instruments, accurate end-of-day proof and handling large volumes of cash.
  
• Provide courteous prompt service and proactively identify member needs, recommend solutions to meet needs and refer members to HVCU platform staff, business partners and automated services. Contribute to branch and individual sales and service goals.
  
• Adhere to branch safety and security practices and procedures. This includes member confidentiality, securely maintaining cash and negotiable items, and physical security standards.
  
• Maintain a working knowledge of HVCU products, services and systems. Utilize resources to ensure quality member service.
  
• Support the successful implementation of marketing programs and promotions by creating member awareness and recommend products when the promotion meets the member’s needs.
  
• Adhere to all Credit Union policies, procedures, and regulatory agency requirements. Participate in all required and recommended training and development including, but not limited to, Bank Secrecy Act training (BSA) and demonstrate attained knowledge. Participate on Credit Union teams, projects and strategic initiatives when the opportunity arises. Perform additional duties and special projects as assigned.
  
• Embrace and apply HVCU’s guiding principles to all activities and responsibilities. • Assesses participant’s physical and functional needs, along with psychosocial status and needs, home safety, family caregiver burden, etc. utilizing professional knowledge, skills of observations and interviewing skills.

• Completes assessment documents, problem lists and interventions and other documentation required for appropriate data collection and analysis as part of a quality assessment and performance improvement program.
• Develops and implements the home care components and works with other team members to develop a comprehensive Interdisciplinary team plan of care based on the needs of participant and caregivers, and goals mutually acceptable to the participant/family and significant others.
• Monitors effectiveness and outcomes regularly and keeps the team informed as to participant progress and level of need. Remains alert to pertinent input from other team members, participants, and family members/caregivers.
• Reviews and revises goals and approaches to member/family care in coordination with interdisciplinary team members.
• Communicates with primary care providers and center nurses as needed to coordinate medication, ancillary tests and functions such as home blood draws as needed.

• Directs the provision of personal care, nursing rehabilitation tasks and other work of Home Health Aides and Geriatric Caregivers in the home.
• Documents changes in the participant’s condition and details care provided by completing all required documentation and ensuring compliance to Agency standards and policies.
• Travels to patients’ homes and/or other facilities using approved transportation options to deliver direct care to the participant as appropriate within the regulations.

• Implements administrative goals and policies and interprets objectives of Nursing Services to the nurses on the units.
• Assist in ensuring appropriate staffing is met during shift.

• Rounds the nursing units daily to ascertain the condition and needs of the residents and informs Director of Nursing of all changes and related problems.
• Observes nursing care of the residents to ensure that orders are carried out as directed and treatment is administered in accordance with physician's instructions.
• Assists with orientating newly employed nurses and clinical placement of nursing students.
• Arranges hospital transfer of residents.
• May render nursing care, administer medication and perform treatments.

• Performs initial and ongoing physical therapy evaluation according to APTA standards of practice and New York State Law and Rehabilitation Department guidelines.
• Provides physical therapy services for the patient, as ordered by the physician.

• Administers speech and language therapy treatments to patients as per physician's orders using modalities and techniques appropriate for the individual patient.
• Makes an initial and periodic evaluation of the patient's speech and language and rehabilitation needs. Sets long and short-term goals in measurable terms and adjusts treatment plans accordingly.
• Consults with the Rehabilitation Therapies Coordinator and nursing staff to make joint decisions regarding continuation of, or dismissal from care when the need for speech therapist treatment is no longer necessary.

The Home Health Aide is responsible for performing health related tasks and may also provide assistance with personal hygiene, housekeeping and other related supportive tasks to a client in his/her home or facility under the supervision of a registered nurse.
