Responsibilities:
- Evaluate program participants through appropriate evaluation procedures and develop written treatment programs for each participant.
- Administer appropriate treatment programs and communicate program participant needs to appropriate staff. Provides needed re-evaluation and programmatic changes as necessary.**
- Maintain timely and appropriate program participant/administrative and department documentation.
- Ensure adherence to accreditation standards, licensure standards, professional code of ethics, and confidentiality.
- Attend programmatic meetings on request for team reports, policy setting, exchange of information, etc.; attends other appropriate meetings/conferences as needed.
- Attend clinical meetings to exchange, discuss, and evaluate program participant treatment programs.
- Serve on rehabilitation treatment team(s). Communicates with physicians, staff, professionals/paraprofessionals, consultants, families, and community resources as needed to ensure effectiveness of physical therapy program.
- Plan and implement in-service programs for staff and other groups as requested/assigned by supervisor.
- Participate in orientation programs for new staff, program participants, and PT/PTA students.
- Write service summaries as requested by case manager/program director and maintains daily billing summaries and turns them into the appropriate staff.
Qualifications:
- Bachelor's Degree in PT, SLP, or OT
- One year’s experience preferred
- State licensure
- Heavy work. Exerting up to 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects.

Adult Community Case Manager:
Description
Independence Association seeks a full-time case manager to join our growing adult case management team. As a case manager, you will play a key role in assisting individuals with intellectual disabilities or autism, by accessing resources to assist them to live a full and inclusive life. You will do this by providing thorough assessments of strengths and needs, comprehensive planning of service needs, advocacy, outreach and resource coordination unique to every person served. You will work closely with both the individual and his or her identified support group, and be a leader in assisting individuals to develop a yearly Person Centered Plan to achieve their goals. Clinical supervision is provided for all pursuing a social worker license, along with a reimbursement program for professional testing and licensing fees.
Job Duties:
- Manage a caseload assigned by team leader and/or program manager.
- Enter all notes of service into the agency electronic medical records system within established agency timelines.
- Coordinate, schedule, facilitate and/or lead the members of their team to create the ISP.
- Assist individuals and their families to make choices, achieve personal goals, and promote the individual’s inclusion into the community as identified in the ISP.
- Coordinate and facilitate transition and discharge planning.
- Assist and monitor individuals’ efforts to secure and maintain needed services and entitlements such as housing, financial benefits, health needs and educational supports.
Requirements:
• Bachelor's degree (required)
• Experience of 1 year in human services field (required)
• Valid driver’s license (required)
• Ability to work independently
• Great written and oral communication skills
• Comfort advocating for individuals with various levels of disability in a variety of settings
Physical Requirements*:
- Ability to drive a vehicle for up to a half an hour commute to the home of family.
- Ability to type on the computer for up to 4 hours per day with breaks.
