As a Family Support Specialist, Foster Care, you will provide support to children in care through transportation, one-on-one respite services and oversee family visits. You will report to the Program Supervisor and work onsite with travel required.

Responsibilities:

• Transportation of children to visits and appointments.
• Oversee family visits using the family visit coaching model.
• One-on-one support for the children.
• Maintain cultural sensitivity, competence, and client confidentiality.
• You will work 40 hours per week with a shifting schedule based on the needs of the program which includes some evenings and occasional weekends.
• Document comprehensive notes on observations and services provided.
• Other responsibilities as assigned.

Requirements and Education:

• High School Diploma required
• 1-2 years of related experience, or equivalent education and experience
• A valid driver's license

Clinician, Families Work

Licensed Social Workers are eligible for a $7,500 retention bonus

As a Clinician in the Families Work program, you will provide six to nine months of intensive services to a caseload of ten families whose children are either at risk of being removed from their homes or are already in placement. The goals of the program are to prevent out-of-home placement for troubled children, provide a safe environment to return children already in placement, and prevent future out-of-home placement. You will report to the Program Coordinator and you will work a combination of on-site and remote with travel to client's homes required.

Responsibilities:

• Provide home and community-based assessment, planning and treatment with individuals, groups and families with those enrolled in our Family Preservation services (the level of intensity / number of direct services hours per family is dependent upon the services in which one is employed)
• Complete necessary all records and client information following agency, county, and state policy and funding requirements
• Provide liaison contact with collateral resources such as referring agencies, schools, family court, and other agency personnel serving on behalf of a child and family as it relates to their treatment
• Ensure provisions of regular treatment plan meetings and services plan reviews.

Requirements and Education:

• Requires a Masters degree in Social Work, Mental Health Counseling or related field
• Minimum of one year of post-graduate experience.
• Must have a valid drivers license.

Available shifts - Monday - Friday between 8:00 am - 5:00 pm - Nights (until 8:00 pm) and Saturdays by appointment only

As a Clinician, you will work as part of an interdisciplinary team with the underserved populations in our community by providing clinical counseling to individuals, families and couples in an outpatient mental health clinic licensed by the New York State Office of Mental Health (OMH). You will report to the Director of the Behavioral Health Center. Sessions may be conducted in the office or via Tele-mental Health as clinically indicated. This an on site position.

Responsibilities:

• Manage a caseload of clients aged birth to 65, providing comprehensive assessment, diagnosis, and individual and family therapy services to those assigned to caseload.
• Create initial treatment plans, subsequent treatment plan reviews, progress notes for each scheduled appointment, and other documentation/assessments as indicated.
• Collaborate with on-site prescriber, other medical and mental health providers, schools and any other community supports for client care.
• Attend Behavioral Health Center staff meeting twice a month.
• Attend and participate in Supervision; weekly, bi-weekly or monthly based on licensure level or individual needs.
• Provide referrals and linkages to community supports, resources, and services.

Requirements and Education:

• Master's degree in Social Work or Mental Health Counselor.
• Experience in human services, case management, line staff at a residential program, or any other related direct care within human services, both pre and post master degree, will be considered.
• License and/or Limited Permit eligible, have a limited permit in discipline or Fully Licensed (LMHC/LCSW/LCSW-R), preferred.
• Good standing with the NYS Office of the Professions.
