As a Licensed Practical Nurse in Foster Care, you will coordinate appropriate medical care, dental care, and mental health services nursing care/services to a youth and adolescent population with emotional, behavioral and physical needs. You will ensure that medical treatment services meet the requirements of the regulating agency. You will report to a Registered Nurse and you will work onsite.
Responsibilities:
- Communicate with medical director/providers regarding observation in client’s health/behavior.
- Provide client and family education regarding medications and treatments.
- Collaborate with our staff regarding concerns and behaviors.
- Participate in Interdisciplinary Medication Review meetings.
- Nursing documentation using EHR.
- Update discharge planning needs.
- See foster children in foster homes as needed.
- Other duties as assigned within scope of practice and based on agency needs.
Requirements and Education:
- LPN Licensed in New York State.
- 1 to 2 years of experience.
- Experience working with youth and adolescent population desired.
- Must pass physical.
- Must have a valid NYS drivers license and reliable transportation.

Teacher Aide
Sign On Bonus - $1,500(paid in 2 parts 1/2 at 30 days and 1/2 at a year) (full time only qualifies)
Available shifts - Monday - Friday between 8:00 am - 4:00 pm - no weekends
Northern Rivers has opportunities for Teacher Aides at our Neil Hellman School. The school serves residential and day students with serious emotional, behavioral and learning challenges, and our team members provide the therapeutic, educational and vocational services they need to empower our students to reach their maximum potential to ensure student success.
For some of these children, coming to school allows them to be in a place where they are cared for and allowed to just be kids, and to reach their educational goals without the stress of food insecurity or housing availability.
As a Teacher Aide, you see the progress a child in your classroom is making as each week advances, giving you a sense of accomplishment knowing that you make a difference in each child’s life.
Teacher Aides assist the classroom teacher in all aspects of behavior management and supporting students' success in school. You will work onsite at our Albany campus, and follow the school calendar, which includes a six-week summer school beginning in July.
This position is the perfect opportunity for someone interested in working with children in a teaching setting, but without the educational criteria of a teacher.
Responsibilities:
Monitor student behavior and help maintain discipline in the classroom; record behavior data as requested; assist with crisis prevention; assist with de-escalation techniques with students.
Help implement behavior plans (BIP); record behavior data; and the completion of incident reports when requested.
Monitor, supervise and assist students during non-instructional periods within a variety of school environments (e.g. lunch, recess/breaks, bus loading/unloading, playground, classroom, washroom, field trips, transportation, assemblies) to maintain a safe and positive learning environment.
Help implement classroom PBIS universal components and Tier 2 and Tier 3 strategies for selected students.
Collaborate with teachers and other staff to incorporate information provided by parents, doctors, nurses, therapists and others to understand the needs of and provide services for students.
Always monitor safety and well-being of students; monitor student attitudes and encourage positive behavior and interactions with peers.
Perform clerical duties; maintain records of student progress; develop and file incident reports; maintain student files; check daily attendance; make copies; develop classroom displays and instructional material; maintain and operate computers, smartboards and other classroom equipment.
Attend meetings, training, professional learning sessions (e.g. first aid, TCI, CPR, emergency procedures, behavior management) to enhance professional skills and competencies.
Follow all Agency policies and procedures, along with state and federal regulations pertaining to our school and students.
Perform other related duties and responsibilities as requested by the teacher and principal

- Work with the direct treatment providers to establish treatment goals for children, utilizing historical and current available information.
- Meet and communicate with biological families, guardians, foster families, and case workers.
- Assess individual and family situations.
- Identify problem areas, treatment goals and methods to attain goals.
- Provide therapeutic services to children on at least a weekly basis and families on at least a bi-weekly basis using differential treatment modalities.
- Work with interdisciplinary team to develop and implement a comprehensive treatment plan and to monitor ongoing progress of child and family.
- Learn and practice case coordination skills.
- Work with referring and collateral agencies and prepare reports and correspondence as needed.
- Record and maintain case record in accord with Agency, OMH, and other state mandates.
