POSITION SUMMARY

As a Transition Coordinator, you will coordinate transition services and instruction to enable students to reach their fullest potential and be able to live, learn and earn in their community as an adult. You will report to the Program Manager and will work onsite.

Responsibilities:

• Work with NHS staff, Clinical staff, Parents/guardians, outside service providers and identified students to:
  • Design a realistic discharge plan that is guided by each youth’s wishes, hopes and dreams
  • Coordinate your client driven discharge planning process with local OCFS, OMH, and NYSED representatives that supports a client-driven plan which integrates housing, health and medical care, education and employment needs.
  • Coordinate a set of vocational and transitional related activities including instruction, community experiences, employment and volunteer experiences, and the acquisition of daily living skills.
  • Examine/identify post secondary vocational training options
  • Respectfully honor each youth’s feelings about past and current relationships and ensure that when needed individuals will have the opportunity to work on issues of grief, loss and enduring relationships
  • Complete a Permanency Pact and identify specific people who agree to offer life long supports (either related or not)
  • Develop a Circle of Supportand identify specific people who agree to advocate for and support the student during the transition process
• Promote active involvement of identified students in planning and implementing the transition component of their IEP.
• Coordinate services to ensure that each identified student has an accurate Work Skills Employability Profile and earns a Regents Certificate of Work Readiness upon graduation from the Neil Hellman School
• Coordinate transition portfolios for each identified student.
• Provide 6 months of follow up and maintain post-NHS discharge statistics for each identified student.
• Attend and participate in all treatment reviews, case reviews and CSE meetings for identified student
• Complete all documentation in a thorough and timely manner

Requirements and Education:

• Bachelor's Degree and at least 3 years of experience working with individuals who demonstrate serious emotional and behavioral challenges.

POSITION OVERVIEW

Clinical Case Manager, Intensive Aftercare Prevention Program

As a Clinical Case Manager in the Intensive Aftercare Prevention Program, you will provide six to nine months of intensive services to families whose children are either at risk of being removed from their homes or are already in placement. The goals of the program are to prevent out-of-home placement for troubled children, provide a safe environment to return children already in placement, and prevent future out-of-home placement. You will report to the Program Coordinator and you will work with a partner (clinician) in a combination of on-site and remote with travel to client's homes required.

Responsibilities:

• Provide home and community services to individuals, groups, and families as prescribed in the treatment plan for those enrolled in our Family Preservation Services.
• Educate families in a variety of social-emotional and parenting skills such as communication, discipline, anger management, emotional regulation, attunement, caregiver affect management, decision making, problem-solving, social skill building, budgeting, family and home organization, behavioral management, etc.
• Work in partnership with a Master’s level clinician to provide needed services to an assigned caseload of families.
• Complete and maintain appropriate case records through the completion of intake, assessments, progress notes, uniform case records, reports, etc. following agency, county, and state policy and funding requirements.
• Participate in the comprehensive assessment and creation of the treatment plan for assigned cases to determine and address service needs and ensure the provision of regular treatment plan meetings and service plan reviews.
• Provide liaison contact with collateral resources such as referring agencies, schools, family courts, and other agency personnel serving on behalf of a child and family as it relates to their treatment.
• You will work Monday through Friday from 8:30 am to 5:00 pm with a flexible schedule that includes evenings and a rotating on-call schedule.

Requirements and Education:

• Bachelor's degree in social work, Sociology, Criminal Justice, Psychology, Human Services, or a related field;
• Experience working with at-risk youth and families;
• Home and community-based experience is a plus.

As a Program Coordinator, you will be responsible for the administrative and programmatic oversight of a Foster Care team. You will oversee staff, the foster families associated with the office and maintain agency resource relationships within the counties the foster care program supports. You will report to the Director of Foster Care and will work onsite.

Responsibilities:

• Supervise multistage foster care treatment teams in areas of intake and ongoing treatment planning for foster youth and foster families.
• Ensure the diligent efforts made to reach permanency of foster children in placement.
• Coordinate the work of a multidisciplinary team comprised of permanency care managers, and clinical and nursing departments to fulfill these goals of permanency, safety, and wellbeing for our children in care.
• Provide regular supervision and complete performance evaluations of program staff such as Permanency Care Managers, Family Support Workers, and Administrative Assistants in the program.
• Work with the Local Department of Social Services to fulfill contracted services.
• Develop and maintain relationships with outside agencies for resources to program.
• Assist in the training and selection of prospective foster parents.

• Ensure case records are maintained to the standards of NY OCFS regulations, COA standards as well as agency standards.
• Collaborate with Northern Rivers service departments including finance and human resources.
• Participate in departmental meetings.

As a Residential Youth Counselor, you will provide daily therapeutic care and programming according to agency policy and governing bodies' (DSS, OMH, JCAHO, etc.) requirements for elementary aged children.. You will work on-site on our Albany campus and will report to the Residential Supervisor.

Provide for the physical and psychological needs of clients, including physical safety, hygiene, clothing, food, cooking, medical care, and providing a reliable and nurturing environment.

• You will follow an individual youth’s 1:1 expectations plan, ICMP, and any safety plans in place.

As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.

Responsibilities:

• Level I - Provides care management services to children with low and medium acuities. The Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in person with the member.
• Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.
  • Engage children, youth, and families with outreach and enrollment activities for successful enrollment into a children's health home
  • Gather and complete assessments and enter documentation that confirms eligibility and acuity, identifying both strengths and areas of need
  • Work with children, youth, families, and service providers to create a comprehensive Plan of Care
  • Identify and engage additional providers (e.g. natural supports, parent advocates, primary care physicians) with youth and families for the execution and continuation of the Plan of Care
  • Convene and conduct Interdisciplinary team meetings to review the Plan of Care; identify progress, and continued needs, and confirm ongoing eligibility
  • Document progress and arrange for transitions between systems, placements, and higher levels of care following changes in eligibility and acuity.
  • Use NY State and Health Home information systems to input required Health Home documentation
  • Comply with Medicaid and program billing regulations

• Develop and implement a classroom instructional plan based on a students IEP and grade and subject area curricula; work with content area teachers to ensure that their lessons are differentiated as to meet the needs of all students.
• Develop and implement Individualized Educational Plans (IEPs) for all students on your caseload. Ensure that accommodations and modifications are provided. Provide specially designed instruction and monitor student progress toward academic and behavioral goals. Complete progress reports and IEP updates, and participate in CSE meetings as scheduled.
• Coordinate the creation of special strategies for reinforcing material or skills based on an understanding of individual students, their needs, interests, and abilities; create and modify materials to match student abilities.
• Develop and coordinate the implementation of single and classroom behavior plans for managing student behavior to maintain discipline in the classroom; oversee the collecting of behavior data; assist classroom staff with crisis prevention and intervention techniques; assist classroom staff with de-escalation techniques; teach students appropriate behavior strategies.

Requirements and Education:

• Requires a Bachelor's degree in Special Education (Master's Degree preferred) or subject area, and ability to work with students who demonstrate serious emotional and behavioral challenges, ages 6-21. General education certification with enrollment in a program that leads to special education certification may be considered.
• Requires that you acquire a NYS certification in Special Education or appropriate subject area, or enrollment in a program that leads to NYS teacher certification within one year of hire

As a School Psychologist, you will be providing school psychology services to students including staff consultation, Positive Behavioral Interventions and Supports (PBIS) coordination, Functional Behavioral Assessments (FBA) and Behavior Intervention Plan (BIP) development; testing; Committee on Special Education (CSE) participation, counseling and progress monitoring. You will report to the Principal and you will work onsite.

Responsibilities:

• Consult with staff regarding the educational, emotional, social, and behavioral needs of the students
• Complete classroom observations and complete FBA’s and BIP’s in collaboration with classroom staff
• Participate in Pre-CSE meetings
• Attend all CSE meetings
• Participate in all phases of PBIS
• Facilitate Tier II and III/FBA/BIP
• Ensure BIP’s are progress monitored
• Facilitate in PBIS data team
• Facilitate in PBIS leadership team
• Complete all psychological evaluations
• Participate in the intake process and complete any necessary testing
• Provide staff training and resources

Requirements and Education:

• A Master’s degree and at least three to four years of related experience, including two years of experience with special education students, FBA/BIP and PBIS experience.

Clinician, Mobile Crisis - Days

As a Clinician for Mobile Crisis, you will be providing crisis risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), crisis stabilization and interventions, de-escalation techniques and follow-up services to the identified client experiencing emotional, behavioral, and psychiatric crises. These interventions, provided by telephone and on-site mobile visits, will be implemented using a strength-based, trauma-informed, systems-oriented perspective and will include the development of crisis prevention and safety plans; follow-up to evaluate the effectiveness of plans; coordination of services with existing treatment providers; and linkage to additional community-based therapeutic resources and supports. You will directly report to the Clinical Site Supervisors with Director and Executive Program Director guidance and support. You will work on-site with travel to the community and individuals' homes.

Responsibilities:

• Provide rapid response to adults, children, adolescents, and families, who are in crisis, experiencing an emotional or behavioral disturbance by phone and on-site within the community, and as assigned, participate in acute care emergency services rotation system designed to facilitate and strengthen community-based crisis intervention services. This includes rotation of schedule to provide coverage during holidays and weekends.
• Conduct risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), mental status examinations, and substance abuse screening.
• Analyze information to identify action plan, which includes crisis stabilization plan, safety, and crisis prevention plans, and plan for follow-up. Consult with team and supervisors.
• Coordinate with community resources for collateral input and follow-up care, including referrals for mental health treatment and substance abuse assessment and treatment.
• Provide short-term crisis intervention and stabilization services to adults, children, adolescents, and their families in the home or community (wherever the individual is experiencing a crisis).

Shift/Schedule:

• Fulltime; 5 days a week 8 hour shifts or 4 days a week 10 hour shifts
• Cross shifts: 10am-6pm, 11am-7pm, or 12pm-8pm
• Part time hours available
• Weekend and holiday expectations
• When scheduled for a weekend shift your schedule will be 5, 8 hour shifts for that week

Requirements and Education:

• Looking for applicants with their LMSW, LMHC, LCSW, LCSW-R, MSW-LP or MHC-LP licensure, must be license eligible and willing to apply for a limited permit in discipline upon hire
• Requires a Masters' degree (MSW/MHC) and two years of related experience
• Willing to pass and obtain licensure within 6 months of hire - Required
• Must have a valid NYS Drivers License and be willing/able to drive to clients' homes and communities under a variety of circumstances and weather conditions. Must be willing to work in many settings, including clients' homes and other community locations where there may be pets, cigarette smoke, etc.
