POSITION OVERVIEW
Residential Case Manager
Our Residential Case Manager will work out of our Albany, NY campus and will be responsible for supporting clients in the Residential System while performing duties in collaboration with a clinician.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Arrange and attend home visits
- Prepare transition plans
- Coordinate Independent Living Services
- Act as the primary contact between Schools and Residential
- Coach family visits
- Complete various assessments, including FASP and SPR
- Complete accurate case notes and timely input the notes into Connections database
- Provide transportation for clients
- Maintain contact with DSS and family
- Function as a contributing member of the interdisciplinary treatment team
- Attend dorm meetings
- Attend treatment review meetings
EDUCATION and/or EXPERIENCE
Requires a Bachelors degree or commensurate level of education and related experience.
Experience with individuals in care systems preferred.
Drivers license is required for this position.
Work requires 25 50% community travel/visits.

As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Responsibilities:

- Complete all required and recommended training
Requirements and Education:
- Level 1 - Requires an Associate's degree in Social Work or a related field, with 2 to 5 years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
- Level 2 - Requires a Bachelor of Arts or Science degree, and two years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.

As a Peer Support Specialist, you will be a member of the Mobile Team, assisting the clinicians on Mobile visits and providing support and advocacy to individuals in crisis. You will also be a resource to the clinicians and case manager, through sharing their knowledge of common challenges individuals with SPMI endure and sharing knowledge about community resources. As a Clinician, you will work as part of an interdisciplinary team with the underserved populations in our community by providing clinical counseling to individuals, families and couples in an outpatient mental health clinic licensed by the New York State Office of Mental Health (OMH). - Master's degree in Social Work or Mental Health Counselor.

Clinical Site Supervisor, Mobile Crisis
Licensed Social Workers are eligible for a $7500 signing bonus!

As a Mobile Clinical Site Supervisor, you will oversee daily management and oversight of all staff to ensure management of the crisis line and onsite response to all 6 counties is coordinated. You will report to the Director of Crisis Services and you will work on-site.
Responsibilities:
- Provide daily management and oversight to staff including:
- Manage the crisis line.
- Monitor triages to ensure appropriate call management response.
- Assist with coordinating team response based on staffing per call.
- Maintain knowledge of the schedule for team response.

- Oversee the training of new staff according to each discipline.

- Maintain knowledge of Human Resources policies and procedures.
- Participate in advanced trainings to the position.
- Complete Performance Evaluations yearly of supervisees.
- Monitor all Documentation of staff as oversite to ensure quality documentation.
- Enhance collaborative teamwork between all staff.
- Cross shifts with evening hours expected to provide support to both shifts across the hours of 7-11pm for Mobile Crisis Services; evening hour coverage responsibility shared with another site supervisor.

As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model.
