Description
As a Case Manager, Regional Permanency Resource Center you will provide supportive services and case management services to adoptive, guardianship and kinship families so they are able to make informed decisions with regards to services and supports that best meet their needs. You will report to the Case Manager Supervisor. You will work in a hybrid environment with some travel to client's homes in the community.
Responsibilities:
- Function as a systems navigator to families who have adopted or who have children in guardianship or kinship care through the provision of case management.
- Meet with families in their home to assess needs
- Assess needs, conduct service planning, service plan implementation, service coordination, monitoring and follow-up, reassessment and review with the family via in-home or community monthly visits. Services include but are not limited to information and referral services; referral to legal services; attendance at family court dates; transportation; housing assistance; childcare; basic family needs (vital needs in emergencies and with expenses that are unforeseen); educational advocacy services; and facilitating appointments with social services, medical health providers, and mental health services.
- Respond to the request for in home support referrals for resources and services
- Provide crisis intervention supports when needed
- Work with family to access needed respite services
- Conduct follow up contacts with families at prescribed intervals
- Participate in family engagement activities
- Co-lead support groups
- Participate in training programs offered by the program as assigned
- Use lived experience to empathize and provide hope. Preference for someone with experience with kinship, adoption or guardianship as a youth or in a parenting role.
- Maintain records including required data collection information according to the program standards and expectations. Case Managers must maintain Evolv record with progress notes and all pertinent documentation, a discharge summary and post-case closure documentation.
- Participate as a trainer in programs in the community, to increase understanding of the needs of families who have adopted or have children in guardianship or kinship care
- Complete risk assessments as required by Northern Rivers Quality Management Department
Requirements and Education:
- Bachelor’s level degree in social work, psychology, or human services, or a bachelor’s level degree in any field with at least two years of case management experience; or an associate’s level degree in psychology or human services with at least two years of case management experience.
- Must be able to communicate effectively and develop positive working relationships
- A valid NYS drivers license

Behavior and Campus Support Worker
You will ensure the security of campus and community residential locations, including working with staff, maintaining communication with resident’s family and guardians, and overseeing resident transportation needs. You will report to the Online/Oncall supervisor and you will work on-site.
Responsibilities:
- Oversee and ensure implementation of all applicable emergency systems, agency policies, and program procedures.
- Respond to crises within assigned Residential Program/locations. Ensure response follows agency policy and protocol appropriate to the situation, and ensure that the individual clients behavior management plan(s) are followed. Ensure any physical intervention is conducted following TCI standards. Ensure completion of all forms and appropriate documentation of every restraint.
- Provide support to teams when asked for behavioral interventions and events. Will direct the team in absence of residential supervisor and will be a source of training , supporting and modeling for other staff.
- Ensure the security of campus and community locations, responding to any security concerns. Conduct random checks of assigned locations to ensure safety of residents.
- Be the primary person in charge of calling the police for any resident considered missing, completing or ensuring completion of the Missing Person Report form and canceling Missing Person Reports upon a residents return.
- Ensure contacts have been made with the residents family and guardian for all significant incidents.
- Maintain regular entries in the Online/On call log regarding all significant events that occur, and provide the leadership with specific additional information as. Ensure completion of documentation for all significant incidents.
- Ensure significant incidents are called into the Justice Center as applicable.
- Serve as the central person in charge of off campus trips. Approve and log all off-campus trips and assure such trips have proper staff coverage.
- Oversee and transport clients for medical, mental health, and other essential needs.
- Check and assure that all visitors have prior approval to be on the grounds.
- Be available to hear and document any neighbor concerns.
Requirements and Education:
- Requires an associate's degree;bachelor's degree and equivalent experience preferred.
- 3 years of experience, or equivalent education and experience.
- Drivers license is required.
- Must be able to pass the required physical.
- Must be able to complete and pass TCI training.

Per Diem Clinician, Permanency Resource Center (PRC)
As a Per Diem Clinician, you will provide trauma-informed family therapy, child therapy, group therapy, and crisis intervention as indicated to individuals, families and groups in a Permanency Resource Center. This can include outpatient individual or family therapy, group therapy, consultation to caregivers, and support to case managers. Work is designed to be short-term but will follow the needs of the family as written in their service plan. You will be assigned between 2 and 15 cases. Therapy is to be provided in the community or in office, and is paid for by the program, not through insurance reimbursement. You will report to the Licensed Clinician and you will work in a hybrid model with community-based travel required.
Responsibilities:
- Provision of outpatient therapy remotely or in person with youth or caregivers within the Permanency Resource Center system.
- Document the contact within the Evolv system.
- Attend and participate in individual supervision on at least a monthly basis and more frequently if the acuity of the assigned cases requires this.
- Provide therapeutic services to individuals assigned to case load.
- Provide case management services as clinically indicated.
- Provide referral, collateral and linkage services.
- Record and maintain case records according to program standards.
Requirements and Education:
- A Master’s degree in one of the healing arts.
- A minimum of a limited permit in social work, family therapy, mental health counseling, or the healing arts.

As a Respite Worker for the Permanency Resource Center, you will provide respite services to youth in adoptive, guardianship and kinship families. Respite may also be available to families as part of the service plan and can occur at parent support groups or other PRC events and will be designed to be consistent with the children's needs and interests. Outings or activities with individual youth will be coordinated with the supervisor and parent. Respite provided as part of a group activity at a support group will be designed to allow parents the opportunity to have time separate from their children while they attend a support group. You will report to the Case Manager Supervisor or designated supervisor at the event. You will work onsite with youth in the community.
Responsibilities:
- Provide supervision for youth while parents attend support group.
- Provide respite as assigned to a youth as part of their service plan which can include activities in the community or in the youth’s home if that is the agreed upon plan.

Job Description:
Information Technology Services (ITS) at the University at Albany seeks applicants to fill vacancies for four Computer Desktop Support Specialist positions. Specialists serve as initial responders to support the integrity, security, reliability, and availability of university-owned computers running Apple, Windows, and Linux operating systems. They work directly with customers to support, diagnose, and resolve service requests and incidents related to all types of desktop and mobile devices, peripherals, AV and recording/broadcasting technology and telephones.
Successful candidates have excellent customer service and communication skills, the ability to work effectively under challenging times and technology constraints, and the knowledge to work with a wide range of hardware and applications. A variety of online and physical tools are used to resolve incidents across all three campuses in faculty/staff offices, conference and classrooms, research labs, and athletic venues. Specialists should enjoy getting out of the office, work equally well independently or as part of a team, and be comfortable directly assisting customers with various levels of technical expertise. A flexible work schedule, including occasional evenings and weekends, is required to support athletics and other events held outside of traditional business hours.
While all Specialists share the competencies outlined in the minimum qualifications, some unique skill sets across the group are desirable. These include specialization in Apple operating systems (macOS, iOS) and endpoints, and expertise in supporting and troubleshooting AV technologies and endpoints as outlined in preferred qualifications. ITS provides training, certification, and other professional development for staff interested in expanding their skills.
Primary Responsibilities
Serve as first responders in support of desktops and mobile devices across a wide range of ITS services:
- Set up and configure university-supported hardware, operating systems, and applications to maintain desktops and mobile devices, peripherals, AV, and recording/broadcasting technology (including cabling, wiring, terminations, networking, and programming), and telephones.
- Assist, diagnose and resolve operating systems (Macintosh, Windows, Linux), applications, and connectivity problems for desktops and mobile devices, peripherals, AV and recording/broadcasting technology, and telephones.
- Perform hardware lifecycle and maintenance service activities for supported offices, classrooms, conference rooms, and telephones.
- Assist with software and applications maintenance, upgrades, and installations as needed.
- Assist, diagnose and resolve incidents and service requests in faculty/staff offices, classrooms, conference rooms, and research labs located across all three campuses using a variety of modes of support (including conferencing tools, in person, or both).
- Provide excellent, timely, and effective customer service.

- Provide on-site primary support for designated Athletics and other University events outside traditional business hours, including evenings and weekends.
Service and work management:
- Contribute to the creation/updating of customer and internal documentation and pertinent workflows.
- Utilize the appropriate tools and apply established best practices to manage day-to-day operations, service activities, and projects.
- Contribute to continuous process and work management improvement by recommending new best practices, efficiencies or changed workflows.
- Ensure established Operating Level Agreements (OLAs) and Service Level Agreements (SLAs) are followed consistently.
- Actively participate, as needed, in ITS projects.
Additional Responsibilities:
- Be available to provide scheduled support and consultation outside normal business hours, including occasional evenings, holidays, or weekends within reasonable obligation and expectation.

Requirements:
Minimum Qualifications:
- A bachelor's degree at time of hire from a college or university accredited by a U.S. Department of Education (DOE) or by an internationally recognized accrediting organization or an associate degree and two years of full-time experience supporting, installing, configuring, and troubleshooting IT equipment.
- Experience troubleshooting and resolving common problems with operating systems (Windows, Macintosh, Linux).
- Experience supporting customers and troubleshooting productivity applications software (i.e., Microsoft Office), or conferencing tools such as Zoom or Teams.
- Experience supporting customers with varying levels of computer expertise from beginner to advanced users.

As a Residential Youth Counselor, you will provide daily therapeutic care and programming according to agency policy and governing bodies' (DSS, OMH, JCAHO, etc.) requirements for elementary aged children.. You will work on-site on our Albany campus and will report to the Residential Supervisor.
Responsibilities:
1:1 Residential Counselor
Provide for the physical and psychological needs of clients, including physical safety, hygiene, clothing, food, cooking, medical care, and providing a reliable and nurturing environment.
You will follow an individual youth’s 1:1 expectations plan, ICMP, and any safety plans in place.
Engage the youth in activities and conversation that are age appropriate and at the appropriate developmental level.
Document in writing a summary of each shift.
Learn the history and specific issues associated with the youth who requires 1:1; will learn how to safely manage any crisis.
Transport 1:1 client for medical, mental health, and other essential needs.
RC1
You will must be able to work within trauma informed framework of program, including identifying and using coping/safety plans, maintaining consistency, providing regular routine, and controlling own response to youth.
Engage and form positive relationship with all youth served.
Complete all mandatory trainings according to programmatic guidelines.
Display basic competency in TCI techniques.
Follow outlined program routines, therapeutic plans of youth, and program rules and protocols.
Work as part of a team including colleague residential counselors, clinicians, on line oncall, supervisors, and other administrative and functional support staff.
Maintain respectful and appropriate communication with internal and external customers, including county workers, medical staff, parents etc.
Complete and maintain documentation to meet programmatic and regulatory standards.
Transport clients for medical, mental health, and other essential needs.
RC2
All of the above plus the following:
You will facilitate groups/coached visitation, understanding and applying curriculum and appropriate practice models, i.e. DBT, SAY, etc.
Take on specific leadership responsibilities for dorm/cottage, including coordinating activities, participating in program-wide committee.
Must have the ability, skill, and knowledge to take a lead role in a team.
Form positive relationships with more challenging youth and families.
RC3
All of the above plus the following:
Display critical thinking and problem solving skills related to residential program needs.
Display good judgment and decision making related to all program routines, therapeutic plans of youth, and program rules and protocols.
Lead, delegate, and plan work for other team members.
Requirements and Education:
HS Diploma or GED is required.
Experience working with emotionally disturbed population.
NYS drivers license with less than 6 points on license.
Must pass the required physical.
Must be able to complete TCI training.

POSITION OVERVIEW
Clinical Case Manager, Intensive Aftercare Prevention Program
As a Clinical Case Manager in the Intensive Aftercare Prevention Program, you will provide six to nine months of intensive services to families whose children are either at risk of being removed from their homes or are already in placement. The goals of the program are to prevent out-of-home placement for troubled children, provide a safe environment to return children already in placement, and prevent future out-of-home placement. You will report to the Program Coordinator and you will work with a partner (clinician) in a combination of on-site and remote with travel to client's homes required.
Responsibilities:
- Provide home and community services to individuals, groups, and families as prescribed in the treatment plan for those enrolled in our Family Preservation Services.
- Educate families in a variety of social-emotional and parenting skills such as communication, discipline, anger management, emotional regulation, attunement, caregiver affect management, decision making, problem-solving, social skill building, budgeting, family and home organization, behavioral management, etc.
- Work in partnership with a Master’s level clinician to provide needed services to an assigned caseload of families.
- Complete and maintain appropriate case records through the completion of intake, assessments, progress notes, uniform case records, reports, etc. following agency, county, and state policy and funding requirements.
- Participate in the comprehensive assessment and creation of the treatment plan for assigned cases to determine and address service needs and ensure the provision of regular treatment plan meetings and service plan reviews.
- Provide liaison contact with collateral resources such as referring agencies, schools, family courts, and other agency personnel serving on behalf of a child and family as it relates to their treatment.
- You will work Monday through Friday from 8:30 am to 5:00 pm with a flexible schedule that includes evenings and a rotating on-call schedule.
Requirements and Education:
- Bachelor's degree in social work, Sociology, Criminal Justice, Psychology, Human Services, or a related field;
- Experience working with at-risk youth and families;
- Home and community-based experience is a plus.
