POSITION SUMMARY

Provide targeted skill building activities with youth in community residence program. Skill training to include areas of independent living and social skill activities.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• At minimum, 50% of work week is spent face to face with clients to provide services designated in the Treatment Plan.
• Provide services based on the youths needs and preferences as identified in the Treatment Plan.

Clinician, Juvenile Fire Safety Services

Full-time Licensed Social Workers are eligible for a $7,500 retention bonus!

As a Clinician in the Juvenile Fire Safety Services, you will provide six to nine months of intensive services to families whose children are either at risk of being removed from their homes or are already in placement. Services are provided to families wherein a youth has engaged in unsanctioned fire-related behavior, there is a suspicion that unsanctioned fire-related behavior has occurred/may occur, or where there is concern that the youth exhibits an elevated interest in fire. The goals of the program are to prevent out-of-home placement for troubled children, provide a safe environment to return children already in placement, and prevent future out-of-home placement. Along with detecting youth likely to be involved in unsanctioned fire-related behavior, educating the youth about fire safety, preventing the development of undetected fire-related behaviors, interrupting dangerous behavioral patterns of using fire as a coping mechanism, and replacing dysfunctional patterns with healthy coping skills. You will report to the Program Coordinator and will work a combination of onsite and remote with travel to client's homes required.

Responsibilities:

• Provide home and community-based assessment, to include fire setting pre-assessment screenings and comprehensive fire setting risk assessments, planning and treatment with individuals, groups and families with those enrolled in our Family Preservation services (the level of intensity / number of direct services hours per family is dependent upon the services in which one is employed).
• Facilitate fire safety educational groups (10-week duration divided into latency-age and adolescent groups).
• Skill building to reduce risk of fire setting behavior, which includes anger management, stress management, problem-solving training and coping skills.
• Complete necessary all records and client information following agency, county, and state policy and funding requirements.
• Provide liaison contact with collateral resources such as referring agencies, schools, family court, and other agency personnel serving on behalf of a child and family as it relates to their treatment.
• Responsible for the clinical decisions when assigned and paired to work as a team with another clinician.
• Ensure provisions of regular treatment plan meetings and services plan reviews.

As a Clinician for Mobile Crisis, you will provide crisis risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), crisis stabilization and interventions, de-escalation techniques and follow-up services to the identified client experiencing emotional, behavioral, and psychiatric crises. You will provide these interventions by telephone and on-site mobile visits and will implement them using a strength-based, trauma-informed, systems-oriented perspective, including the development of crisis prevention and safety plans; follow-up to evaluate the effectiveness of plans; coordination of services with existing treatment providers; and linkage to additional community-based therapeutic resources and supports. You will directly report to the Clinical Site Supervisors with Director and Executive Program Director guidance. You will work on-site with travel to the community and individuals' homes.

Responsibilities:

• Provide rapid response to adults, children, adolescents, and families, who are in crisis, experiencing an emotional or behavioral disturbance by phone and on-site within the community, and as assigned, participate in acute care emergency services rotation system designed to facilitate and strengthen community-based crisis intervention services. This includes rotation of schedule to provide coverage during holidays and weekends.
• Conduct risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), mental status examinations, and substance abuse screening.
• Analyze information to identify action plan, which includes crisis stabilization plan, safety, and crisis prevention plans, and plan for follow-up. Consult with team and supervisors.
• Coordinate with community resources for collateral input and follow-up care, including referrals for mental health treatment and substance abuse assessment and treatment.
• Provide short-term crisis intervention and stabilization services to adults, children, adolescents, and their families in the home or community (wherever the individual is experiencing a crisis).

Schedule/Shift:

• Fulltime; 5 days a week 8 hour shifts or 4 days a week 10 hour shifts
• Weekend and holiday expectations
• When scheduled for a weekend shift your schedule will be 5, 8 hour shifts for that week

As an Online/Oncall, you will ensure the security of campus and community residential locations, including working with staff, maintaining communication with resident’s family and guardians, and overseeing resident transportation needs. You will report to the Online/Oncall supervisor and you will work on-site.

Responsibilities:

• Oversee and ensure implementation of all applicable emergency systems, agency policies, and program procedures.
• Respond to crises within assigned Residential Program/locations. Ensure response follows agency policy and protocol appropriate to the situation, and ensure that the individual clients behavior management plan(s) are followed. Ensure any physical intervention is conducted following TCI standards. Ensure completion of all forms and appropriate documentation of every restraint.

• Maintain regular entries in the Online/On call log regarding all significant events that occur, and provide the leadership with specific additional information as. Ensure completion of documentation for all significant incidents.
• Ensure significant incidents are called into the Justice Center as applicable.
• Serve as the central person in charge of off campus trips. Approve and log all off-campus trips and assure such trips have proper staff coverage.
• Oversee and transport clients for medical, mental health, and other essential needs.
• Check and assure that all visitors have prior approval to be on the grounds.
• Be available to hear and document any neighbor concerns.

As a Case Manager, Regional Permanency Resource Center you will provide supportive services and case management services to adoptive, guardianship and kinship families so they are able to make informed decisions with regards to services and supports that best meet their needs. Responsibilities:

• Function as a systems navigator to families who have adopted or who have children in guardianship or kinship care through the provision of case management.

As a Residential Youth Counselor, you will provide daily therapeutic care and programming according to agency policy and governing bodies'. Our Hospital to Home (H2H) program provides a structured, supportive transition for youth ages 11 to 17 with acute mental health needs who are discharging from psychiatric hospitalization or emergency room stays. Through a multi-phase care model, the program aims to reduce unnecessary readmissions and long-term placements by offering comprehensive and community-based supports. You will work on-site on our Schenectady campus and will report to the Residential Supervisor.

Responsibilities:

Provide for the physical and psychological needs of clients, including physical safety, hygiene, clothing, food, cooking, medical care, and providing a reliable and nurturing environment.

1:1 Residential Counselor

• You will follow an individual youth’s 1:1 expectations plan, ICMP, and any safety plans in place.

• Engage the youth in activities and conversation that are age appropriate and at the appropriate developmental level.

• Document in writing a summary of each shift.

As a Preschool Special Education Teacher, you will develop and implement of classroom instructional plans for preschool students in the Neil Hellman Preschool. Responsibilities:

• Follow all Agency policies and procedures, along with state and federal regulations about our school and students.
• Develop and implement a classroom instructional plan based on a student's IEP and Creative Curriculum Objectives for Development and Learning, creating lessons that are differentiated as to meet the needs of all students.
• Develop and implement Individualized Educational Plans (IEPs) for students in their classroom. Ensure that accommodations and modifications are provided. Clinician, Mobile Crisis - Days

As a Clinician for Mobile Crisis, you will provide crisis risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), crisis stabilization and interventions, de-escalation techniques and follow-up services to the identified client experiencing emotional, behavioral, and psychiatric crises. You will provide these interventions by telephone and on-site mobile visits and will implement them using a strength-based, trauma-informed, systems-oriented perspective, including the development of crisis prevention and safety plans; follow-up to evaluate the effectiveness of plans; coordination of services with existing treatment providers; and linkage to additional community-based therapeutic resources and supports. You will directly report to the Clinical Site Supervisors with Director and Executive Program Director guidance. You will work on-site with travel to the community and individuals' homes.

Responsibilities:

• Provide rapid response to adults, children, adolescents, and families, who are in crisis, experiencing an emotional or behavioral disturbance by phone and on-site within the community, and as assigned, participate in acute care emergency services rotation system designed to facilitate and strengthen community-based crisis intervention services. This includes rotation of schedule to provide coverage during holidays and weekends.
• Conduct risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), mental status examinations, and substance abuse screening.
• Analyze information to identify action plan, which includes crisis stabilization plan, safety, and crisis prevention plans, and plan for follow-up. Consult with team and supervisors.
• Coordinate with community resources for collateral input and follow-up care, including referrals for mental health treatment and substance abuse assessment and treatment.
• Provide short-term crisis intervention and stabilization services to adults, children, adolescents, and their families in the home or community (wherever the individual is experiencing a crisis).

Schedule/Shift:

• Fulltime; 5 days a week 8 hour shifts or 4 days a week 10 hour shifts
• Weekend and holiday expectations
• When scheduled for a weekend shift your schedule will be 5, 8 hour shifts for that week

As a Licensed Practical Nurse, you will coordinate appropriate medical care, dental care, and mental health services nursing care/services to a youth and adolescent population with emotional, behavioral and physical needs. You will ensure that medical treatment services meet the requirements of the regulating agency. You will report to a Registered Nurse and you will work onsite.

Responsibilities:

• Complete the nursing observations report findings to the registered nurse
• Review lab and test results, reporting critical values to RN and providers
• Medication management including ordering medications and obtaining prior authorizations
• Dispense medications
• Communicate with medical director/providers regarding observation in client’s health/behavior
• Provide client and family education regarding medications and treatments
• Collaborate with our staff regarding concerns and behaviors
• Participate in Interdisciplinary Medication Review meetings
• Nursing documentation using EHR
• Update discharge planning needs
• Other duties as assigned within scope of practice and based on agency needs 

Requirements and Education:

• LPN Licensed in New York State
• 1 to 2 years of experience
• CPR Certification
• Experience working with youth and adolescent population desired 
• Must pass physical.
• Must be able to complete TCI training.

As a Residential Supervisor, you are responsible for supervising staff who provide daily therapeutic care and programming according to agency policy and governing body standards and requirements. You'll report to the Program Manager and work on-site.

Responsibilities:

• Assist the Program Manager / Assistant Director in creating an energetic, creative, multi-disciplinary team of staff person to work cohesively and therapeutically with the youth and their families on the unit
• Design, implement and supervise program activity which will lead to the attainment of goals described within treatment plans
• Supervise and evaluate the work of residential staff assigned to residential programs.
• Maintain records as required
• Participate in interviewing and selection of new staff members
• Participate in training of residential staff
• Conduct residential staff meetings
• Understand and apply the general framework and skill building attached to various treatment models used with the youth, including DBT, CBT, Motivational Interviewing, ARC as a member of an interdisciplinary treatment team
• Follow, review and implement all agency policy and procedures to ensure all regulations are followed by employees.
• Make the unit a family friendly place, where family members feel welcome and activities are planned that include family members. Teach, and then ensure, that staff persons treat family members with respect and listen to their concerns, document them, and pass them on to the appropriate clinician and administrator.
• Maintain the physical order of the building and therapeutic environment of the units
• Develop and maintain schedules within the assigned campus residential unit
• Participate in the evaluation of youth referred to the Agency for residential care
• Participate in the appropriate on call system
• Oversee and/or transport clients for medical, mental health, and other essential needs
• Work with clinical staff to coordinate the implementation of treatment plans in the milieu
• Assist in crisis intervention as needed

Requirements and Education:

• Associate’s degree required, bachelor’s degree preferred 
• 3 years of experience working with vulnerable populations/ adolescents required.
• Must have supervisory experience or demonstrated ability to manage a residential unit.
• Required to maintain TCI certification.
• A valid driver’s license and meet agency insurability standards for using agency vehicles and transporting youth.
• Must pass physical.

These interventions, provided by telephone and on-site mobile visits, will be implemented using a strength-based, trauma-informed, systems-oriented perspective and will include the development of crisis prevention and safety plans; follow-up to evaluate the effectiveness of plans; coordination of services with existing treatment providers; and linkage to additional community-based therapeutic resources and supports. You will directly report to the Clinical Site Supervisors with Director and Executive Program Director guidance and support. You will work on-site with travel to the community and individuals' homes.

• Provide rapid response to adults, children, adolescents, and families, who are in crisis, experiencing an emotional or behavioral disturbance by phone and on-site within the community, and as assigned, participate in acute care emergency services rotation system designed to facilitate and strengthen community-based crisis intervention services. This includes rotation of schedule to provide coverage during holidays and weekends.
• Conduct risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), mental status examinations, and substance abuse screening.
• Analyze information to identify action plan, which includes crisis stabilization plan, safety, and crisis prevention plans, and plan for follow-up. Consult with team and supervisors.
• Coordinate with community resources for collateral input and follow-up care, including referrals for mental health treatment and substance abuse assessment and treatment.
• Provide short-term crisis intervention and stabilization services to adults, children, adolescents, and their families in the home or community (wherever the individual is experiencing a crisis).

Requirements and Education:

• Looking for applicants with their LMSW, LMHC, LCSW, LCSW-R licensure
• Requires a Masters' degree (MSW/MHC) and two years of related experience
• Must have a valid NYS Drivers License and be willing and able to drive to clients' homes and communities under a variety of circumstances and weather conditions. Clinician, Families Work

Licensed Social Workers are eligible for a $7,500 retention bonus

As a Clinician in the Families Work program, you will provide six to nine months of intensive services to a caseload of ten families whose children are either at risk of being removed from their homes or are already in placement. The goals of the program are to prevent out-of-home placement for troubled children, provide a safe environment to return children already in placement, and prevent future out-of-home placement. You will report to the Program Coordinator and you will work a combination of on-site and remote with travel to client's homes required.

Responsibilities:

• Provide home and community-based assessment, planning and treatment with individuals, groups and families with those enrolled in our Family Preservation services (the level of intensity / number of direct services hours per family is dependent upon the services in which one is employed)
• Complete necessary all records and client information following agency, county, and state policy and funding requirements
• Provide liaison contact with collateral resources such as referring agencies, schools, family court, and other agency personnel serving on behalf of a child and family as it relates to their treatment
• Ensure provisions of regular treatment plan meetings and services plan reviews.

Requirements and Education:

• Requires a Masters degree in Social Work, Mental Health Counseling or related field
• Minimum of one year of post-graduate experience.

Clinician, Foster Care - Per Diem

As a Foster Care Clinician, you will provide clinical services to children in foster care and support foster families, and families within the foster care system. The Therapeutic Foster Family Program provides foster and pre-adoptive foster homes for abused and neglected children from birth through age 21. Each foster parent receives intensive training and ongoing professional support to promote a positive, successful experience. You will report to the Clinical Supervisor and you will work onsite.

Responsibilities:

• Provide assessment, treatment planning and general counseling for children in foster care.
• Provide individual, group, and family therapy to children and families, when needed
• Conduct specialized risk assessments
• Consult with other staff involved in the care of the child and families
• Attend all case related meetings (Counsel on Special Education, Termination of Parental Rights, staff meetings, service provider meetings, emergency meetings, court), for cases they are serving
• Participate in formal and informal supervision.
• Meet regularly in the community regarding cases they are seeing for general counseling.
• Have regular communication/meeting with birth families around progress/problems of children in care and how to support them in and work towards permanency.
• Provide regular case documentation for all children and families on workload including assessments, progress notes, Family Service Plan, etc.

Requirements and Education:

• Requires a Master’s degree in counseling with related experience.
• Limited permit is required; professional licensure (LMSW, LCSW, LMHC, LMFT, LCAT) preferred.

• Masters' degree (MSW/MHC) and two years of related experience required
• LMSW, LMHC, LCSW, LCSW-R, MHC – LP or MSW-LP licensure required
• MSW -LP holders must be license eligible and willing to apply for a limited permit upon hire.
• MHC- LP holders must already have obtained their limited permit and be close to finishing required clinical hours
• Willing to pass and obtain licensure within 3 months of hire - Required

• Create initial treatment plans, subsequent treatment plan reviews, progress notes for each scheduled appointment, and other documentation/assessments as indicated.
• Collaborate with on-site prescriber, other medical and mental health providers, schools and any other community supports for client care.
• Attend Behavioral Health Center staff meeting twice a month.
• Attend and participate in Supervision; weekly, bi-weekly or monthly based on licensure level or individual needs.
• Provide referrals and linkages to community supports, resources, and services.

Requirements and Education:

• Master's degree in Social Work or Mental Health Counselor.
• Experience in human services, case management, line staff at a residential program, or any other related direct care within human services, both pre and post master degree, will be considered.
• License and/or Limited Permit eligible, have a limited permit in discipline or Fully Licensed (LMHC/LCSW/LCSW-R), preferred.

As a Residential Youth Counselor, you will provide daily therapeutic care and programming according to agency policy and governing bodies' (DSS, OMH, JCAHO, etc.) requirements for elementary aged children.. You will work on-site on our Albany campus and will report to the Residential Supervisor.

Provide for the physical and psychological needs of clients, including physical safety, hygiene, clothing, food, cooking, medical care, and providing a reliable and nurturing environment.

1:1 Residential Counselor

• You will follow an individual youth’s 1:1 expectations plan, ICMP, and any safety plans in place.

• Engage the youth in activities and conversation that are age appropriate and at the appropriate developmental level.

• Document in writing a summary of each shift.

• Learn the history and specific issues associated with the youth who requires 1:1; will learn how to safely manage any crisis.

• Seek and obtain training in any particular area specific to the youth who requires 1:1care.

Clinician, Positive Youth Program

As a Clinician in the Positive Youth Progression program, you will maintain a caseload of eight to ten youth and meet with each weekly. Positive Youth Progression provides six to nine months of home and community-based prevention services to youth who are at risk of being removed from their homes. The goals of the program are to prevent out-of-home placement for troubled youth and to avoid the juvenile justice system. You will report to the Program Coordinator and you will work with a partner (Clinical Case Manager) in a combination of on-site and remote with travel to client's homes is required (travel reimbursement offered).

Responsibilities:

• Complete assessment to identify the strengths and needs of the youth, caregiver and family.
• Develop treatment plans for the youth and families to help identify and meet their goals.
• Provide home and community-based clinical treatment coupled with supportive and skill based wrap around casework interventions based on assessment, planning and treatment with youth and their families
• A trauma-informed approach to service delivery and working with the clinical case manager to conduct family sessions
• Communicate with the youth’s family to monitor progress and support the family’s effort to keep the youth safe
• Ensure provisions of regular treatment plan meetings and services plan reviews to assess on going progress and identify any needed modification to treatment planning
• Coordinate services with service providers and collaterals involved with the youth and their family
• Complete paperwork within agency, count, and state guidelines.
• You will work Monday-Friday with flexible schedule that includes evenings and a rotating on-call schedule.

Your Experience and Qualification:

• Masters degree in social work, mental health counseling, or equivalent from an accredited graduate school program
• 0-2 years post-masters experience
• Experience working with at-risk youth and their families preferred
• Home and community based experience is a plus
