Description

As a Family Advocate in the mobile crisis department you will be a member of Mobile Crisis Services. You will assist the clinicians on Mobile visits by providing support and advocacy for family members of the child or adolescent in crisis. You will be a resource to the program by sharing knowledge of common challenges families have experienced with a mental health diagnosis or other identified disabilities and sharing knowledge about communities resources. You will report to the Clinical Site Supervisor and work onsite.

Responsibilities:

• Respond to onsite crisis call with clinicians to assist with crisis planning and stabilization.
• Help with follow-up calls to families, on the crisis call and telephonically
• Collaborate with service providers involved with the children/adolescents and their families.
• Provide information and peer to peer support to families who are served by the program
• Educate and empower families regarding their roles in developing crisis plans and crisis prevention with their children/adolescents.
• Ensure families are satisfied with the services they are receiving and, if not, help them advocate for what they need and how to advocate.
• Be a liaison to other local supports in the community.
• Function within a diverse treatment team and maintain work relationships with team members, agency colleagues, external providers and clients (respect amongst disciplines, trauma-informed practice)
• Understand the program mission and expectations of the county funding sources
• Remain in compliance with mandatory agency and program trainings and additional trainings, as assigned
• Use clinical team members for support regarding client contact
• Attend all regular staff meetings
• Participate in supervision and annual job performance evaluation

Requirements and Education:

• High School Diploma or GED required.
• 1+ years of related experience required.

As a Youth Peer Advocate, you will amplify the youth's voices and choices. You will provide support and advocacy for young people enrolled in the OMH Community Residence: Miriam House and the Residential Treatment Facility. You will work parallel to the Family Peer Advocate and support the youth in their recovery to return to their home communities.

You will work with youth in many settings which include: home, school, and community. You will inspire hope and offer support to the youth and help communicate with others, advocate for their needs, build on strengths to achieve their goals, provide role modeling and coaching and self-care. You will work closely with the youth, the family, and both internal and external treatment team members. You will report to the Program Coordinator at the Miriam House and work onsite with community-based travel.

Responsibilities:

• Assist youth in advocating for service needs and benefits
• Assist youth in unmet needs
• Advocate by being the voice for the youth and represent the youth in meetings related to and about the youth
• Empower youth by sharing your own lived experience
• Advocate, especially in the absence of the youth, for youth and keep in mind the youth when plans are being discussed or established
• Engage the youth through outreach and support
• Empower youth through education
• Promote wellness through modeling and coaching
• Provide mutual support, hope, and reassurance which includes your own personal recovery story as the you deem appropriate and beneficial and share your recovery with the parents/caregivers as a means to engage them
• Connect the youth to community resources and services.
• Accompany youth to appointments and meetings
• Support youth to develop networks of support
• Help youth develop self-advocacy skills
• Maintain all documentation and tracking promptly
• Attend regular supervision, staff meetings, and other meetings
• Follow the team philosophy/approach in working with youth, families, and staff
• Follow agency Compliance Plan and Code of Conduct

Requirements and Education:

• A High School Diploma or GED required
• A valid NYS Driver's License required
• Certification as a NY State Peer Specialist/Peer Advocate required

Family Peer Advocate

As a Family Peer Advocate, you will help children with emotional and mental health challenges through support and advocacy with parents and caregivers. You will report to the Program Manager, Community Solutions and Peer Support and you will work onsite with travel required.

Responsibilities:

• Provide support, advocacy and resources to the family based on the child's needs as identified by the parent/caregiver
• Provide services within a Family-driven Care framework, including respecting family choice and voice, believing in each family’s potential to learn and grow, and utilizing strategies that empower families
• Work with family members to identify goals and objectives of treatment
• Educate parents/caregivers on how to advocate if services are not meeting their needs
• Participate in relevant treatment meetings and attend all required trainings
• Complete all documentation

Requirements and Education:

• Family Peers have identified lived experience as a parent or primary caregiver of a child with mental health/behavioral health/special education challenges.
• Provisional Family Peer Credential required, with Professional Family Peer Credential to be obtained within 18 months.
• Minimum of HS Diploma or equivalency.
• Valid NYS Driver's License required

As a Peer Support Specialist, you will be a member of the Mobile Team, assisting the clinicians on Mobile visits and providing support and advocacy to individuals in crisis. You will also be a resource to the clinicians and case manager, through sharing their knowledge of common challenges individuals with SPMI endure and sharing knowledge about community resources. You will be an individual (adult) with lived experience with mental illness. You will report to the Executive Program Director and you will work on-site.

Responsibilities:

• Collaborate with service providers involved with individuals with SPMI.

• Provide information and peer to peer support to individuals who are served by the program.

• Educate and empower clients and their supports regarding their roles in developing crisis plans and in crisis prevention.

• Educate service providers regarding an individual's role in developing a crisis plan for themselves and in crisis prevention. Ensure that the individual is an integral part of the crisis planning process.

• Inform service providers (including Mobile Crisis and PCFC providers) regarding person- centered and strength-based practices. Maintain and model a focus on strengths.

• Advise Clinical Supervisor.

• Ensure that clients are satisfied with the services they are receiving and, if not, help them advocate for what they need.

• Educate individuals and primary caretakers on how to advocate if services are not meeting their needs.

• Be the liaison to other local supports in the community.

• Link individuals to Peer Support Services activities such as support groups, social events, newsletter.
