As a Director of Community Transition Time Program (CTTP), you will oversee a new model of care for youth to keep them out of unnecessary hospital settings. You will manage a multi-disciplinary team that provides services and support to adolescent youth and their families in the community and short-term residential settings. Responsibilities will include the management of the program, staff matters, client referrals, client treatment, financial viability, marketing of the program, interfacing with referral sources including local hospitals and county mental health providers, program manual, and program systems. You will report to the Senior Director of Residential Services and will work onsite.
Responsibilities:
- Lead on recruiting, hiring and onboarding team members.
- Provide overall supervision to the team.
- Monitor caseload size and coordinates program intakes and discharges.
- Provide training and supervision for the Staff.
- Oversee a short-term eight bed residential unit that provides 24/7 care.
- Make rotating on-call schedule for after hours and weekend coverage by team members.
- Become subject matter expert in this new program model and participate in collaborative meetings with the state and other agencies.
- Educate the community about Community Transition Time and Intervention program, outcomes, theoretical background.
- Develop and maintain collaborative relationships with other agencies, schools, colleges, institutions, hospitals, primary care providers, housing authorities, and law enforcement agencies to help meet the needs of this population through timely referrals, connections and warm handoffs.
- Ensure staff are adhering to the fidelity of the program.
- Participate in all required trainings as directed.

POSITION SUMMARY
Provide targeted skill building activities with youth in community residence program. Skill training to include areas of independent living and social skill activities.
ESSENTIAL DUTIES AND RESPONSIBILITIES

As a Clinician for Mobile Crisis, you will provide crisis risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), crisis stabilization and interventions, de-escalation techniques and follow-up services to the identified client experiencing emotional, behavioral, and psychiatric crises. - Provide rapid response to adults, children, adolescents, and families, who are in crisis, experiencing an emotional or behavioral disturbance by phone and on-site within the community, and as assigned, participate in acute care emergency services rotation system designed to facilitate and strengthen community-based crisis intervention services. This includes rotation of schedule to provide coverage during holidays and weekends.
- Conduct risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), mental status examinations, and substance abuse screening.
- Analyze information to identify action plan, which includes crisis stabilization plan, safety, and crisis prevention plans, and plan for follow-up. Consult with team and supervisors.

The Residential Supervisor in charge will ensure the security of campus and community residential locations, including working with staff, maintaining communication with resident’s family and guardians, and overseeing resident transportation needs. Description
As a Clinician for Mobile Crisis, you will provide crisis risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), crisis stabilization and interventions, de-escalation techniques and follow-up services to the identified client experiencing emotional, behavioral, and psychiatric crises. These interventions, provided by telephone and on-site mobile visits, will be implemented using a strength-based, trauma-informed, systems-oriented perspective and will include the development of crisis prevention and safety plans; follow-up to evaluate the effectiveness of plans; coordination of services with existing treatment providers; and linkage to additional community-based therapeutic resources and supports. You will directly report to the Clinical Site Supervisors with Director and Executive Program Director guidance and support. You will work on-site with travel to the community and individuals' homes.
Responsibilities:
- Provide rapid response to adults, children, adolescents, and families, who are in crisis, experiencing an emotional or behavioral disturbance by phone and on-site within the community, and as assigned, participate in acute care emergency services rotation system designed to facilitate and strengthen community-based crisis intervention services. This includes rotation of schedule to provide coverage during holidays and weekends.
- Conduct risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), mental status examinations, and substance abuse screening.
- Analyze information to identify action plan, which includes crisis stabilization plan, safety, and crisis prevention plans, and plan for follow-up. Consult with team and supervisors.
- Coordinate with community resources for collateral input and follow-up care, including referrals for mental health treatment and substance abuse assessment and treatment.
- Provide short-term crisis intervention and stabilization services to adults, children, adolescents, and their families in the home or community (wherever the individual is experiencing a crisis).
Requirements and Education:
- Looking for applicants with their LMSW, LMHC, LCSW, LCSW-R licensure
- Requires a Masters' degree (MSW/MHC) and two years of related experience
- Must have a valid NYS Drivers License and be willing and able to drive to clients' homes and communities under a variety of circumstances and weather conditions. Must be willing to work in many settings, including clients' homes and other community locations where there may be pets, cigarette smoke, etc.

Description
This position may be offered on a hybrid basis depending on case load locations. The current foster care home roster includes clients in Washington, Warren, and Saratoga counties. The Permanency Care Manager is expected to visit various locations on a rolling basis.
As a Case Manager, you will work with the children in care, the Department of Social Services, foster families, and birth families in the journey toward achieving permanency in multiple settings. You will report to the Program Coordinator and work on-site with some travel.
Responsibilities:
- Work with foster and birth families to coordinate all appointments for children, including providing transportation if another plan cannot be found.
- Attend all client-related meetings, including Counsel on Special Education, Termination of Parental Rights, staff meetings, service provider meetings, emergency meetings, court, and others.
- Facilitate meetings with other service providers.
- Support birth families including providing supervised visitation and feedback on parenting skills and communication regarding progress/problems of children in care and obstacles toward permanency.
- Participation in treatment and discharge planning.
- Regular case documentation.
Requirements and Education:
- Associate's Degree and two year of experience in social services, or equivalent education and experience.
- Ability to work night and weekend hours and be part of an on-call rotation to support foster families in after-hours crisis.
- Driver's license and reliable, safe, and clean transportation and a willingness to transport clients in own vehicle. Mileage and expense reimbursement eligible.

As a Clinician, you will work as part of an interdisciplinary team with the underserved populations in our community by providing clinical counseling to individuals, families and couples in an outpatient mental health clinic licensed by the New York State Office of Mental Health (OMH). You will report to the Director of the Behavioral Health Center. Sessions may be conducted in the office or via Tele-mental Health as clinically indicated. This is an on site position
Responsibilities:
- Manage a caseload of clients aged birth to 65, providing comprehensive assessment, diagnosis, and individual and family therapy services to those assigned to caseload.
- Create initial treatment plans, subsequent treatment plan reviews, progress notes for each scheduled appointment, and other documentation/assessments as indicated.
- Collaborate with on-site prescriber, other medical and mental health providers, schools and any other community supports for client care.
- Attend Behavioral Health Center staff meeting twice a month.
- Attend and participate in Supervision; weekly, bi-weekly or monthly based on licensure level or individual needs.
- Provide referrals and linkages to community supports, resources, and services.
Requirements and Education:
- Master's degree in Social Work or Mental Health Counselor.
- Experience in human services, case management, line staff at a residential program, or any other related direct care within human services, both pre and post master degree, will be considered.
- License and/or Limited Permit eligible, have a limited permit in discipline or Fully Licensed (LMHC/LCSW/LCSW-R), preferred.
