Responsibilities

Responsibilities:

• Responsible for providing home care nursing assessment and interventions along with care planning and coordination of 24 hour care for to PACE participants.
• Plan and supervise the personal care and ADL assistance tasks of Geriatric caregivers in the home.
• Conduct visits to participant homes, inpatient facilities and other locations as required to assess participant level of care needs, coordinate with participant, family caregivers, other providers and the IDT.

Job Type: Full-Time

Salary: $60 - $70 per hour

Qualifications

Qualifications:

• NYS Licensed Registered Nurse
• 3 years of Home Care experience
• 1 year supervisory experience

Education:

• Bachelors Degree

• Assist in ensuring appropriate staffing is met during shift.

• Rounds the nursing units daily to ascertain the condition and needs of the residents and informs Director of Nursing of all changes and related problems.
• Observes nursing care of the residents to ensure that orders are carried out as directed and treatment is administered in accordance with physician's instructions.
• Assists with orientating newly employed nurses and clinical placement of nursing students.
• Arranges hospital transfer of residents.
• May render nursing care, administer medication and perform treatments.

• Assists in planning, developing, organizing and implementing departmental goals and objectives.
• Ensure appropriate staffing levels are met for each unit by coordinating with Staffing Coordinator
• Exhibits creativity and initiative in pursuing organizational and departmental growth and development.

Responsibilities:

The ADON partners with the DON to direct nursing activities throughout the skilled nursing facility.

• Assumes the responsibilities of the Direct of Nursing in their absence and assists the Director in the administration, continuity and supervision of nursing care, and guidance of the activities within the nursing department.
• Assists in maintaining a physical, spiritual, social, psychological, and safe environment conducive to the best interests and welfare of the staff and residents.
• Ensures a high level of care is provided to all residents.
• Performs rounds on staff and residents.
• Maintains nursing skills and knowledge of nursing theory.

Job Type: Full-Time

Salary:$102,500 to $128,164 per year

• 5+ years nursing experience
• 2+ years’ experience in nursing administration
• NYS RN License
