Responsibilities
Responsibilities:
Provides initial and ongoing participant assessment, care plan development and implementation of plan in the clinic, group home, nursing home, or home.
- Responsible for full assessment of participants including obtaining health records, performing physical and mental evaluations, evaluating diagnostic tests, and reporting findings and identifying status changes.
- Performs complete analysis of participants including recognizing abnormal findings in testing, exercising clinical judgement and interpreting screenings and other diagnostic tests.
- Responsible for managing tasks such as monitoring on going and chronic health problems in collaboration with physicians, prescribing and adjusting medication within current practice guidelines.
- Monitoring participants care and evaluating effectiveness of individual care plans while making appropriate revisions.
- Functions as a member of the interdisciplinary team and participates in development of comprehensive plan of care and ongoing monitoring of participants health status.
- Participates in clinical administrator on-call rotation.
Job Type:Full-Time
Salary: $140,000 to $160,000 per year

Responsibilities
Responsibilities:
- Responsible for the overall coordination and implementation of the Infection Control Program in the facility.
- Assures a high quality of resident care by identifying areas which need special monitoring programs and initiating and maintaining these programs so that infection hazards will be minimized or eradicated.
- Assists the Director of Nursing in surveillance, epidemic investigation, in service and quality assurance as part of the facility’s infection control programs , to include making rounds on nursing units to evaluate residents and environment problem areas. Monitoring isolation practice and following through on all suspected infections and communicable diseases. Compiling statistics relevant to the infection control report to the Quality Assurance Committee.
- Acts as a liaison with staff and administration in matters relevant to infection control and environmental health and safety.
- Assists in the development and execution of vaccination programs for employees and residents to minimize spread of infections throughout the facility.
- Responsible for the reporting information required by the City or State Health Departments, the center for Disease Control, or other official bodies.
- Performs other related duties as assigned.
Salary: $105,000 - $110,000 per year

- Participates in the development and revision of the participant's plan of care as a member of the interdisciplinary team. Integrates the primary care treatment plan into the plan of care developed by the interdisciplinary team. Interacts with other team members to meet emergent and acute needs of participants. Participates in discharge planning.
- Documents changes in the participant's condition and details care provided by completing all required documentation.
- Collaborates with nurse, nurse practitioner and other physicians to ensure comprehensive and continuous care.
- Assists in monitoring the medical care of enrolled members who are hospitalized in out-of-network and out-of-service area facilities and assists in their transfer to in-network facilities when medically appropriate.

- Attends and participates in scheduled staff meetings and participant care meetings as requested.
- Maintains applicable licensure and certification and pursues professional growth through continuing education process.
