POSITION SUMMARY
As a Transition Coordinator, you will coordinate transition services and instruction to enable students to reach their fullest potential and be able to live, learn and earn in their community as an adult. You will report to the Program Manager and will work onsite.
Responsibilities:
- Work with NHS staff, Clinical staff, Parents/guardians, outside service providers and identified students to:
- Design a realistic discharge plan that is guided by each youth’s wishes, hopes and dreams
- Coordinate your client driven discharge planning process with local OCFS, OMH, and NYSED representatives that supports a client-driven plan which integrates housing, health and medical care, education and employment needs.
- Coordinate a set of vocational and transitional related activities including instruction, community experiences, employment and volunteer experiences, and the acquisition of daily living skills.
- Examine/identify post secondary vocational training options
- Respectfully honor each youth’s feelings about past and current relationships and ensure that when needed individuals will have the opportunity to work on issues of grief, loss and enduring relationships
- Complete a Permanency Pact and identify specific people who agree to offer life long supports (either related or not)
- Develop a Circle of Supportand identify specific people who agree to advocate for and support the student during the transition process
- Promote active involvement of identified students in planning and implementing the transition component of their IEP.
- Coordinate services to ensure that each identified student has an accurate Work Skills Employability Profile and earns a Regents Certificate of Work Readiness upon graduation from the Neil Hellman School
- Coordinate transition portfolios for each identified student.
- Provide 6 months of follow up and maintain post-NHS discharge statistics for each identified student.
- Attend and participate in all treatment reviews, case reviews and CSE meetings for identified student
- Complete all documentation in a thorough and timely manner

Director of Equal Opportunity and Title IX Coordinator
Pay Status and Classification: Exempt, full-time
Supervisor: Vice President and Chief of Staff
Position Purpose: Under general direction by the Vice President and Chief of Staff, Office of the President, works collaboratively with various departments, including Student Affairs, Human Resources, Academic Affairs, Campus Safety and the Chief Diversity Officer to ensure the College’s adherence to Title IX of the Education Amendments of 1972, the Violence Against Women Act (VAWA) Amendments to the Clery Act, New York State Education Law Article 129-B, and other federal and state laws governing gender-based misconduct, discrimination, harassment and equal opportunity. Plays a pivotal role in fostering and securing a nurturing environment conducive to academic and personal growth of all College community members.
Essential Responsibilities and Duties:
- Compliance: Facilitates compliance with all procedures, requirements and timeframes outlined in the College’s Title IX Policy, Gender-based Misconduct Policy, and the Policy Prohibiting Discrimination, Harassment and Retaliation in Employment. Leads an effort to review and revise policies in response to changes in governing laws, regulations, and guidance. Reviews and compiles reports in compliance with federal and state law and to identify and address any patterns or systemic problems of reports of harassment and misconduct within the College community. Completes annual assessment report of progress to goals.
- Process Oversight: Oversees and coordinates investigations, responses, and resolutions to complaints under the Title IX Policy, the Gender-based Misconduct Policy, and the Policy Prohibiting Discrimination, Harassment and Retaliation in Employment. Assures that investigation and resolution processes are conducted in a fair, timely, impartial, respectful, and thorough manner.
- Collaboration & Partnership: Collaborates with colleagues in Student Affairs and Human Resources to identify topics, design and deliver student and employee training including compliance content training (e.g. NCAA Title IX requirements, new student orientation requirements, student training pursuant to New York Education Law Article 129-B, etc.). Coordinates with local organizations and service providers to ensure our community members have access to confidential resources. Manages partnerships with other external agencies with which the College has a partnership in executing its responsibilities under Title IX and other applicable laws and regulations.
- Supervision: Selects, trains, assigns and oversees designated roles including Deputy Title IX Coordinators, investigators, adjudication panel members, appellate panel members and advisors; ensures all are adequately trained in compliance with federal and New York State law.
Qualifications:
- A Master’s degree in a related field such as human resources, law, social justice, higher education administration or a relevant discipline and/or a Juris Doctorate, and a minimum of three years of experience in equal opportunity compliance, Title IX administration, or a related field, preferably in a higher education or nonprofit organizational setting.
- Extensive experience working on compliance with Title IX, Title VII, ADA, Age Discrimination in Employment Act, Section 504 and related regulatory requirements.
- Certification and/or training as required by federal and New York State law to serve in the position, or ability to obtain such certification and training prior to commencement of employment.
- Demonstrated expertise in federal and state laws and regulations pertaining to equal opportunity, affirmative action, discrimination and harassment, including comprehensive knowledge of Title VII, Title IX, the Clery Act, New York State Education Law Article 129-B or similar state laws, and related guidance from the United States Department of Education and applicable state authorities.
- Ability to work outside of regular business hours when necessary.
- Demonstrated understanding and sensitivity towards experiences and expressions of trauma caused by sexual violence, discrimination and harassment.
- Strong presentation skills with an emphasis on education and training for a variety of constituencies.
- Experience conducting and/or overseeing investigations into allegations of discrimination, harassment, or sexual misconduct, and familiarity with principles of procedural fairness and due process.
- Excellent interpersonal, communication, and conflict resolution skills, with the ability to interact effectively with individuals from diverse backgrounds and perspectives.

Compensation
The annual salary range this position is $115,000.00 - $123,000.00. Except for roles with a set rate of pay, the wage/salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The stated hiring rate/range represents the College’s good faith and reasonable estimate of the rate/range of possible compensation at the time of posting.

As a Foster Care Home Finder, you will recruit, train, certify, and maintain foster and pre-adoptive foster homes to children from birth through age 21 who have been removed from their homes and placed in foster care. As a Program Coordinator, you will be responsible for the administrative and programmatic oversight of a Foster Care team. As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. School Based Clinician
The School Based Clinician is responsible for providing clinical services to youth and their families according to program and regulatory guidelines, ensuring that all services are strengths-based, individualized, and comprehensive according to need.
RESPONSIBILITIES:

As a Special Education Teacher, you will develop and implement classroom instructional plans for students and support students success in academic and behavioral goals. You will report to the Assistant Principal and you will work onsite.
Responsibilities:
- Follow all Agency policies and procedures, along with state and federal regulations about our school and students.
- Develop and implement a classroom instructional plan based on a students IEP and grade and subject area curricula; work with content area teachers to ensure that their lessons are differentiated as to meet the needs of all students.
- Develop and implement Individualized Educational Plans (IEPs) for all students on your caseload. Ensure that accommodations and modifications are provided. Custodian - Team Leader (Bilingual in Spanish)
Position purpose: Oversees the custodial staff and ensures that assigned campus facilities are maintained in a clean, safe, and orderly condition. Supervises custodian staff, performs cleaning duties, and maintains high standards of cleanliness. Coordinates work schedules, assigns tasks, and provides training to custodial team members. Provides translation needs for all Custodial teams.

Essential Duties and Responsibilities:
- Supervision and Leadership: Leads, supervises, and coordinates the activities of the custodial team to ensure all assigned tasks are completed. Assesses job performance of custodians for yearly evaluation. Prioritizes/schedules work to be done by custodians. Ensures custodians have completed their assigned tasks in each area.
- Cleaning and Maintenance: Performs regular cleaning duties including sweeping, mopping, vacuuming, etc. Utilizing cleaning equipment such as butler van, ride on scrubbers, and advanced cleaning equipment. Conducts regular inspections of all areas ensuring areas are cleaned and maintained to the highest standards. Provides coverage for absent staff to maintain cleaning standards.
- Communication and Collaboration: Communicates effectively with the Supervisor of Building Maintenance and other staff members about cleaning schedules, and needs. Resolves any custodian related concerns promptly. Pitches in to assist colleagues as needed.
- Operations: Monitors and manages inventory of cleaning supplies and equipment. Executes special cleaning tasks as requested, beyond routine daily cleaning duties. Stays informed about standards for the use, handling, and disposal of cleaning chemicals. Supports the work request system's administrative functions related to cleaning tasks.
- Administration: Processes monthly requests for cleaning supplies. Approves bi-weekly time sheets for all applicable custodians. Attends seminars and other courses of study designed to improve job performance. Complies with departmental and college policies and procedures. Schedules all vacation and absence time off for their team. Schedules appropriate staffing for major events such as ReUnion, Commencement, Trustees, etc.
Qualifications:
- High school diploma or equivalent.
- A minimum of three years of custodial experience. A minimum of three years of supervisory experience within a custodial environment is preferred.
- Fluent in both English and Spanish, with excellent verbal and written communication skills in both languages.
- Ability to work a flexible schedule, including evenings, weekends, and special events.
- Thorough knowledge of cleaning chemicals, proper handling, and disposal procedures.
- Must have a valid New York State Driver's License with a clean driving record.
- Strong organizational and leadership skills.
- Excellent communication and interpersonal abilities.
- Physical requirements: Ability to perform physical tasks for extended periods, including standing, walking, and bending, as well as the capability to lift, carry, and move objects weighing up to 50 pounds regularly.

Compensation
The rate for this position is $20.00 per hour. As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.
Responsibilities:
- Level I - Provides care management services to children with low and medium acuities. The Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in person with the member.
- Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.
- Engage children, youth, and families with outreach and enrollment activities for successful enrollment into a children's health home
- Gather and complete assessments and enter documentation that confirms eligibility and acuity, identifying both strengths and areas of need
- Work with children, youth, families, and service providers to create a comprehensive Plan of Care
- Identify and engage additional providers (e.g. natural supports, parent advocates, primary care physicians) with youth and families for the execution and continuation of the Plan of Care
- Convene and conduct Interdisciplinary team meetings to review the Plan of Care; identify progress, and continued needs, and confirm ongoing eligibility
- Document progress and arrange for transitions between systems, placements, and higher levels of care following changes in eligibility and acuity.
- Use NY State and Health Home information systems to input required Health Home documentation
