The Behavioral Health program and the Youth Assertive Community Treatment (ACT) program are seeking a Psychiatric Nurse Practitioner for a shared role working between our Schenectady NY location and our Rensselaer NY location. Education and/or Experience:
- Requires Board Certification by the American Board of Psychiatry and Neurology. Must have a minimum of one year experience working with youth and adults diagnosed with mental illness.
- Must be fully licensed Nurse Practitionerin the State of New York.

- National Provider Identification (NPI) - issued by the federal Center for Medicare and Medicaid Services (CMS). All electronic prescriptions in NYS require an NPI. Claims processing functions also require the NPI of nurse practitioners and physicians.

As a Program Supervisor, you will be responsible for the administrative and programmatic oversight of a Foster Care team. You will oversee staff, the foster families associated with the office and maintain agency resource relationships within the counties the foster care program supports. Credible Messenger Counselor
As a Credible Messenger Counselor, you will develop transformative mentoring relationships with youth in the justice system. Credible Messenger Mentoring for youth in the juvenile justice system is a transformative process through which individuals from similar backgrounds, especially men and women who have personal experience in the justice system, engage youth in structured and intentional relationships that help them change their attitudes, belief, and actions. - Maintain a caseload of nine youth and and meet with each youth weekly in their home or a mutually agreed upon community setting, as well as providing outreach phone calls and support to caregivers.
- Advocate and support youth as they interact with justice systems, education, employment, health, and social service systems.
- Participate in Credible Messenger training and consultation opportunities to develop restorative justice and mentoring skills, as well as learning about promoting a lifestyle of sustained transformation.

Description
As a Community Service Provider, you will provide home and community-based services to help children with emotional and mental health challenges. You will travel locally to work with children and families in their communities providing a core of rehabilitative services to ensure success, including non-licensed counseling and support for youth and family, skill building, peer support, caregiver support and advocacy, pre-vocational services and respite. You will report to a Community Service Provider Supervisor, and work onsite within the community, as travel is required and documentation may be completed remotely.
Responsibilities:
- Provide services within a Family-driven Care framework, including respecting family choice and voice, believing in each family’s potential to learn and grow, and utilizing strategies that empower families
- Work with family members to identify goals and objectives of treatment
- Provide services based on the needs as identified in the Treatment Plan
- Coordinate with the child and caregiver on the scheduling and provision of services
- Participate in relevant treatment meetings and attend all required trainings
- Complete all documentation
Requirements and Education:
- Bachelor's degree in a related field and at least two years of related experience OR
- Master's degree in Social Work, counseling, psych, or a related human service and at least one year of related experience
- Valid NYS Driver's License required

Description
As a Licensed Practical Nurse, you will coordinate appropriate medical care, dental care, and mental health services nursing care/services to a youth and adolescent population with emotional, behavioral and physical needs. You will ensure that medical treatment services meet the requirements of the regulating agency. You will report to a Registered Nurse and you will work onsite.
Responsibilities:
- Complete the nursing observations report findings to the registered nurse
- Review lab and test results, reporting critical values to RN and providers
- Medication management including ordering medications and obtaining prior authorizations
- Dispense medications
- Communicate with medical director/providers regarding observation in client’s health/behavior
- Provide client and family education regarding medications and treatments
